摘  要:目的通过比较1.44mm与1.06mm脉冲Nd:YAG激光器对犬小肠的切割与穿孔时间,评价新波长1.44mm脉冲激光手术刀的汽化切割特性。方法切割试验:两种激光器采用频率20Hz,单脉冲能量240mJ与600mJ二种剂量,对犬1.0cm长一段肠管进行扫描照射,测量完全切透时间。穿孔试验:两种激光器采用频率5Hz、脉宽640ms、单脉冲能量800mJ一种剂量,对小肠进行垂直照射,记录穿透时间。结果1.44mm激光器比1.06mm激光器汽化好、速度快、创面炭化少、无活动出血。1.44mm脉冲激光器切割与穿孔时间均短于1.06mm脉冲激光器,且存在统计学上的极显著差异(P<0.01)。结论1.44mm相比1.06mm脉冲Nd:YAG激光器有较强汽化切割与穿孔能力。提示1.44mm脉冲激光器作为激光手术刀用于空腔脏器肿物介入切除,切割效率远高于1.06mm激光刀,具有很好临床应用的开发前景。Objective To evaluate the cutting capability of the new wavelength pulse Nd:YAG laser at 1.44mm, by comparing the time taken to cut and perforate the small intestine with 1.44mm and 1.06mm Nd:YAG laser. Methods In the experiment of laser cutting, two given length (1.0cm) in the small intestine was selected to be cut with the 1.44mm or 1.06mm Nd:YAG laser with different pulse energy at 240mJ and 600mJ and with a frequency of 20Hz. In the experiment of perforation, the small intestine was vertically irradiated with the two kinds of laser and the parameters was: frequency 5Hz, pulse duration 640ms and pulse energy 800mJ. The time taken for the cutting and perforation was recorded. Results The time taken for both cutting and perforation with 1.44mm laser was less than that with 1.06mm laser(P〈0.01). Conclusion The cutting or perforating capability of the pulsed 1.44mm Nd:YAG laser is higher than that of the 1.06mm laser. And this suggest that as a special Scalpel for the resection of vacant viscera tumor, the 1.44mm Nd:YAG laser would be far more efficiently than the 1.06mm laser, and the hemostatic effect be satisfactory. The clinical prospect is definite.
