摘  要:目的研究国产医用新型波长(1.44μm)脉冲Nd:YAG激光治疗机对犬膀胱组织作用的量效关系,以评价该机在膀胱手术治疗中的应用前景。方法采用脉宽640μs、频率5Hz、最大单脉冲能量1J的1.44μm脉冲Nd:YAG激光治疗机,按单脉冲能量0(对照)、0.2J、0.5J、0.8J,每次照射脉冲数1、5、10、20、40、80进行动物分组,每组5只比格犬。对犬的膀胱后壁进行活体照射。标本经组织切片制作,显微镜观察,计算机成像与测量,获取汽化宽度、汽化深度、凝固宽度与凝固深度等数据,经统计学处理,绘制量效关系曲线,分析作用机制。结果试验可观测到各剂量组组织效应表现,最大汽化带宽度是1098.5±74.0μm,最大凝固带宽度是2078.2±70.9μm,最大汽化带深度是514.4±19.8μm,出现在单脉冲能量0.8J、脉冲数/次80剂量组;最大凝固带深度是512.7±11.6μm,出现在单脉冲能量0.8J、脉冲数/次20剂量组。结论国产新型1.44μm脉冲激光治疗机对犬膀胱粘膜作用的组织效应程度与单脉冲能量、每次照射脉冲数呈正相关趋势,该机对膀胱组织有较强汽化作用,凝固层足以保证试验中创面清洁、粘膜无活动出血。提示该国产治疗机具有临床膀胱手术的良好开发意义。Objective To study the relationships between doses and effects of the new wavelength (1.44 μ m) pulse Nd:YAG laser radiation on Bladder wall in dogs. And to evaluate the clinical prospect of the laser treatment apparatus in blader surgery. Methods A 1.44μ m pulse Nd:YAG laser treatment apparatus with a pulse duration of 640μs, a frequency of 5Hz and a pulse energy upto 1J was used in this experiment to five beagle dogs. The bladder wall in each dog was irradiated with different pulse energy (0.2J, 0.5J and 0.8J) and with different pulse number (1,5,10,20,40 and 80 pulses), which produced 18 different lesioning data points and 1 normal control(0J and 0 pulse ) points. After radiations the coronal plane sections of every lesioning point were made. By the means of taking photomicrograph, the width and depth of the histological zones of the lesions including photoevaporated zones and coagulation necrosis zones were measured. And then the curve of relationships between doses and effects was drawn. Results The width of the photoevaporated zone was 1098.5 ±74.0 μ m; the width of the coagulation necrosis zone was 2078.2±70.9 μ m; and the depth of the photoevaporated zone was 514.4 ± 19.8μ m, which were at the L18(0.8J of the pulse energy and 80 pulses);, the depth of the coagulation necrosis zone was 512.7 ± 11.6 μ m, which was at the L15(0.8J of the pulse energy and 20 pulses). Conclusion The histologic effects on bladder wall in dogs is positive tendency with the pulse energy and the pulse number of the new 1.44 μ m Nd:YAG laser treatment apparatus produced in China. The bladder wall can be effectively vaporized by the studied laser and at the same time the coagulation necrosis zones is enough for hemostasia,ensuring a clean wound. It is suggested that the clinical prospect in bladder surgery is definite.
