摘 要:目的探讨凋亡相关蛋白Fas、Bcl-2在胎盘滋养叶细胞中的表达及其意义,滋养叶细胞凋亡与Fas、Bcl-2两种蛋白的关系,从而于细胞凋亡的角度探讨重度子痫前期的发病机制。方法用免疫组织化学SP法检测重度子痫前期患者54例(试验组),同时选择同期正常晚孕妇女20例(对照组)做胎盘组织中Fas、Bcl-2蛋白的表达及滋养叶细胞凋亡检测。结果细胞凋亡相关蛋白Bcl-2、Fas在重度子痫前期及正常孕妇胎盘组织中均有表达。其中Bcl-2蛋白的表达在试验组低于对照组(P<0.05);而Fas蛋白的表达在试验组明显高于对照组(P<0.05);在试验组及对照组均可观察到细胞凋亡现象,在滋养层细胞(合体细胞滋养层及细胞滋养层)、间质细胞、血管内皮细胞胞浆及细胞核内均有表达,尤以合体细胞最多见。试验组细胞凋亡较对照组明显增多(P<0.05)。重度子痫前期时,Bcl-2与Fas蛋白的表达呈负性相关;Bcl-2蛋白表达与胎盘细胞凋亡呈负性相关,而Fas蛋白的表达与胎盘细胞凋亡呈正性相关(P<0.01)。结论重度子痫前期的发病原因可能与胎盘滋养叶细胞凋亡及Bcl-2、Fas基因异常表达有关。Objective To investigate the expression and significance of Fas and Bcl-2 in placental trophoblast cells in patients with severe preeclampsia and to explore the pathogenetic mechanism of severe preeclampsia in order to guide clinical treatment. Methods The expression of Fas and Bcl-2 in placental tissue was detected by immunohistochemical staining in 54 patients with severe preeclampsia (trial group) and 20 cases of normal pregnancy (control group), and the apoptosis in trophoblast cells was detected by TUNEL method and confocal laser scanning microscope with fluorescence-FTTC labeling. Results The expression of Fas and Bcl-2 was found both in PIH and control group. The expression of Bcl-2 in trial group was significantly lower than that of control group ( P 〈 0. 05), while the expression of Fas in trial group was significantly higher than that of control group ( P 〈 0.05). The cell apoptosis was found in both groups,however, the cell apoptosis was significantly increased in trial group( P 〈 0.05). The expression Bcl-2 was negatively correlated with that of Fas in trialgroup( P 〈 0. 05 ). The expression of Bcl-2 was negatively correlated with placetal cell apoptosis( P 〈 0.01 ), however, the expression of Fas was positively correlated with placetalcell apoptosis ( P 〈 0.01). Conclusion The pathogenesis of severe preeclampsiamay be relatetd with apoptosis of placental trophoblast cells and the abnormal expression of Bcl-2 and Fas.
