114例伴体重指数异常的寻常型银屑病患者的临床特点分析

摘　　要：目的探讨114例伴体重指数(BMI)异常的寻常型银屑病患者的临床特点。方法对397例18岁以上的寻常型银屑病患者进行问卷调查及体检,并对其中245例进行期银屑病患者的病情严重程度进行评价。计算BMI,将患者分为BMI正常组(BMI<25)与BMI异常组(BMI≥25),比较男性和女性患者中BMI≥25与BMI<25患者的年龄、发病年龄、病程、病情严重程度、家族遗传史。结果397例寻常型银屑病患者中,114例伴BMI异常,占28.7%。男性银屑病患者肥胖发生率与女性无显著差异(P>0.05),超重发生率显著高于女性(P<0.01)。男性BMI≥25组年龄大于BMI<25组(P<0.05),但发病年龄和病史与BMI<25组无显著差异(P>0.05);女性BMI≥25组的年龄、发病年龄、病史均大于BMI<25组(P<0.05)。245例进行期的寻常型银屑病患者中,BMI≥25组的病情较BMI<25组严重,且在女性患者中更明显(P<0.05)。寻常型银屑病患者家族史对BMI没有影响(P>0.05)。结论伴有BMI异常的寻常型银屑病患者往往年龄大、发病年龄迟、病史长、病情重,且在女性患者中更明显。Objective To explore the clinical features of psoriasis vulgaris in patients with abnormal body mass index （BMI）. Methods Three hundreds and ninety-seven patients with psoriasis vulgaris were investigated by questionnaire and physical examination, among them 245 patients at the progressive stage were selected for evaluating the degree of severity. The following items were compared between BMI≥25 group and BMI〈25 group of patients： patients＇ age, the age of onset, course of disease, degree of severity and family heredity. ResuRs In 397 patients with psoriasis vulgaris, 114 （28. 7%） had abnormal BMI. The incidence of overweight in male patients was significantly higher than that in females （P〈0.01）, while no significant difference was found in obesity （P〉0.05）. In the male patients, the average age in BMI≥25 group was higher than in BMI〈25 group （P〈0. 05）, hut no significant difference was found in the initial onset age and course of disease between the two groups （P〉0.05）. For the female patients, the subjects in BMI≥25 group had higher mean age and initial onset age, and longer diseace course than those in BMI〈25 group （P〈0. 05）. In 245 patients at the progressive stage, the lesions observed were severer in BMI≥25 group than in BMI〈25 group, especially in female patients （P〈0. 05）. It seemed that the familial history of psoriasis vulgaris throw no influence on BMI （P〉0. 05）. Conclusion The psoriasis vulgaris patients with abnormal BMI always have higher average age and initial onset age, longer clinical history and severer symptoms, especially in the females.
