摘 要:目的:研究腋淋巴结阴性乳腺癌(ALNNBC)血管内皮生长因子(VEGF)血清浓度与乳腺癌微转移关系。方法:采用巢式RT-PCR方法,检测62例ALNNBC外周血人类乳腺珠蛋白(hMAM)mRNA的表达,阳性表达者判定为乳腺癌微转移;ELISA方法检测患者VEGF血清浓度。分析hMAM mRNA表达及VEGF血清浓度与相关临床病理因素间的关系,VEGF血清浓度与微转移及复发转移的关系。结果:ALNNBC中c-erbB2阳性表达者hMAM mRNA阳性表达率高(P<0.05);hMAM mRNA阳性表达提示乳腺癌存在微转移;ER阴性、P53阳性、c-erbB2阳性、hMAM mRNA阳性表达组VEGF血清浓度高(P<0.05或者P<0.01);高VEGF浓度组无复发生存率较低(P<0.05)。结论:ALNNBC外周血血清VEGF高浓度与微转移及临床复发和转移密切相关,血清VEGF浓度可能是ALNNBC预后预测因子。Objective: To detect the correlation of the serum concentration of vascular endothelial growth factor (VEGF) and micrometastasis in axillary lymph node negative breast cancer (ALNNBC) patients. Methods: Blood samples obtained from 62 ALNNBC were screened for hMAM mRNA by a nested RT-PCR assay and it was regarded as micrometastasis when hMAM mRNA was positively expressed. The serum concentrations of VEGF were examined by ELISA. The expression of hMAM and serum concentrations of VEGF were analyzed when the patients had different clinical and pathological characters, and the relationship between them was also assessed, respectively. Results: The expression of hMAM was higher in c-erbB2 positive group (P〈0.05) while it was not correlated with menopausal status, tumor size, histological type, ER status, arid p53 expression. The positive expression of hMAM in patients' serum might imply the existing of micrometastasis of cancer. It was found that the serum concentrations of VEGF were significantly higher in groups of negative ER, positive p53, positive c-erB2 and micrometastatic patients (positive hMAM) (P〈0.05 or P〈0.01). Disease free survival rate in higher VEGF concentration group was significantly lower than that in lower group (P〈0.05). Conclusion: High concentra-tion of serum VEGF of peripheral blood in ALNNBC correlates with micrometastasis and relapse of breast cancer, and it may be a prognosis factor.
