银杏叶滴丸治疗多梗死性痴呆的疗效机制

摘　　要：目的评价银杏叶滴丸治疗多梗死性痴呆(MID)的临床疗效及其可能的作用机制。方法92例MID患者随机分为两组,治疗组(52例)应用银杏叶滴丸,对照组(40例)应用复方丹参片治疗。在治疗前及治疗后4周分别对两组临床疗效、精神状态检查(MMSE)、Barthel指数评分、长谷川智力量表、血液流变学、血脂及血管内皮细胞功能进行比较。结果治疗组总有效率为63.5%,对照组总有效率为37.5%(P<0.05);治疗组治疗后4周的MMSE评分、Barthel指数评分和长谷川智力量表评定与对照组比较均有统计学意义(P<0.05);治疗组治疗后患者的全血黏度、红细胞比容和血浆纤维蛋白原含量与对照组比较下降明显(P<0.05)。治疗组治疗后血脂各项指标、血清一氧化氮(NO)和内皮素(ET)含量与对照组治疗后比较有统计学意义(P<0.05或P<0.01)。用药后两组患者均未发现明显不良反应。结论银杏叶滴丸治疗MID的临床疗效肯定,无毒副反应;其作用机制可能与其改善患者血管内皮细胞功能、调节血脂和血液流变学等有关。Objective To investigate the therapeutic effect of ginkgo hiloba leave dropping pill on multi- infarct dementia （MID） and its mechanism. Methods Ninety- two patients were randomly divided into two groups：Treatment group （n= 52） treated with ginkgo biloba leaves dropping pills and control group （n=40） treated with compound salvia tablets for four weeks respectively. The clinical therapeutic effects, the scores of mini- mental state examination （MMSE）, Barthel index, Hasegawa dementia scale（HDS）, hemorheology indexes,serum lipid,nitric oxide and endothelin were determined. Results The total clinical effective rate in the treatment group and control group was 66.0% and 37.5% respectively （P〈0.05）. Following four weeks treatment,the scores of MMSE, Barthel Index, HDS, hemorheology indexes, levels of serum lipid, nitric oxide and endothelin in treatment group were improved significantly as compared to the control group （P〈0.05 or P〈0.01）. There was no severely side effect in the trial. Conclusion The ginkgo biloba leave dropping pill was effective for MID, which might be related to promote the recovery of blood hemorheology, serum lipid and vascular endothelium function.
