445对不孕夫妇精液质量情况及相关社会因素分析

摘　　要：目的通过分析445对不孕夫妇精液质量及其相关社会因素,探讨影响不孕的危险因素。方法2008年1月—7月在上海瑞金医院生殖医学中心就诊的不育孕夫妇445对,先分析原/继发、不孕年限等基本情况以及精液质量各指标之间的相关情况,再根据男方精液质量情况分为3组,即精液正常组(83例)、少弱精子症组(344例)和无精子症组(18例)。采用病例-对照研究方法,回顾性分析3组患者男方年龄、体重指数、原/继发、不孕年限、吸烟及精索静脉曲张等因素,采用R×C表卡方检验进行统计比较。结果(1)原/继发不孕患者男性年龄及不孕年限存在差别;(2)精液质量各指标之间存在一定的相关关系,精子密度与A级、B级、C级、D级精子百分率明显相关;(3)与断发不孕相比,原发不孕与精液质量关系更为密切,精索静脉曲张也是影响精液质量的重要因素,吸烟、体重指数与不孕年限对精液质量影响不大。结论应提倡适龄婚育,降低不孕风险,如出现不孕情况,尽早到不孕专科检查治疗。Objective To explore the effect of social factors on the quality of semen of infertile couples. Methods Total 445 pairs of infertile couples from out-patient of Reproductive Medicine Centre of Ruijin Hospital, from February of 2008 to July of 2008, were analyzed in the study. According to the semen quality, all patients were divided into three groups such as normal （83 cases）, oligoasthenospermatism （344 cases） and azoospermia （18 cases）. A retrospec- tive analysis was used to investigate the male＇s age,Body weight index, original/secondary, infertile period, smoking and of varicocele among three groups. Results （1）There were significant differences in males＇age and infertile period between original infertile couples and secondary infertile couples; （2）There exsited a remarkable correlation between density of sperm and motility; （3）In comparison with secondary infertility, primary infertility was a risk factor for the quality of semen, and varicocele was also a risk factor.whereas smoking, body weight index and infertility period were almost no effect on the quality of semen. Conclusion In order to reduce the ocurrence of infertility, goverment should encourage the new couples to get pregnancy at the right age.
