高脂血症对胶原代谢的影响及调脂药物的干预

摘  要:目的:研究高脂血症对胶原代谢的影响,以及调脂药物对胶原代谢的作用。方法:选择单纯脂代谢异常的患者67例,分别按胆固醇、三酰甘油和二者均升高分为3组,其中,高胆固醇血症组(高TC组)20例,每晚口服氟伐他汀40mg;高三酰甘油血症组(高TG组)20例,每日早空腹口服微粒化非诺贝特200mg;混合型高脂血症组(高TC、TG组)27例,联合应用氟伐他汀和微粒化非诺贝特,用法同前。分别于治疗前、治疗4,8周后测定Ⅲ型前胶原(PC-Ⅲ)浓度,治疗过程中监测药物不良反应。同时选年龄、性别相匹配的健康人20例作为正常对照组(正常组),入选时仅采空腹静脉血1次,观察指标同治疗组。结果:治疗前高TC、TG组、高TC组、高TG组与正常组比较,PC-Ⅲ浓度明显增加;血脂异常组各组之间比较,高TC、TG组上指标明显高于高TC组、高TG组;治疗组4,8周与治疗前血脂指标明显降低,PC-Ⅲ浓度均降低,且治疗8周均低于治疗4周。结论:血脂升高可以诱导胶原合成增加,调脂药物不仅能有效地降低血脂,而且能够抑制胶原合成。OBJECTIVE To study the influence of hyperlipidemia on collagen metabolism and therapeutic effect of lipid-adjusting drugs. METHODS 67 dyslipidoses patients without any other disease were enrolled. In these patients,20 patients with only hypercholesterolemia(high TC group)received 40 mg fluvastatin perday. 20 patients with only hypertriglycerindemia(high TG group)received 200 mg micronized fenofibrate perday. 27 Patients with mixed hyperlipidemia(high TG,TC group)received 40 mg fluvastatin and 200 mg micronized fenofibrate together perday. The fasting vein blood sample were acquired before treatment,at &week treatment, 8-week treatment respectively. The plasma concentration of procollagen-Ⅲ(PC-Ⅲ), triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) were measured. Atherosclerosis index( AI) was calculated as (TC-HDL-C)/HDL-C. 20 health people were also enrolled as normal group. RESULTS The baseline data in three treatment groups compared with normal group, The plasma concentration of PC-UI and AI were all higher than those in normal group. In high TG,TC group, the plasma concentration of PC-Ill at baseline was also higher than those in the other treatment groups respectively. In all three treatment groups,after 4 and 8 weeks after treatment, the plasma concentration of PC-Ⅲ were all decreased compared with baseline data, the data at 8 weeks were also lower than those at 4 weeks respectively. Pearson analysis showed that the plasma concentration of PC-Ⅲ was positive related toTG, TC, LDL-C, AI, negative related to HDL-C. CONCLUSION Plasma concentration of PC-Ⅲ increased in patients with hyperlip- idemia. Fluvastatin, fenofibrate can decrease the composition of PC-Ⅲ.
