摘 要:目的研究卡巴胆碱对大鼠腹腔粘连的干预作用及其对粘连组织转化生长因子-β(1TGF-β1)表达的影响。方法22只雄性Wistar大鼠随机分为假手术组(n=6)、腹腔粘连组(n=8)、卡巴胆碱干预组(n=8,50μg/kg)。采用开腹后无菌干纱布摩擦大鼠盲肠蚓突盲端、并损伤侧腹壁法制作大鼠腹腔粘连动物模型。各组于术后第14天处死动物,参考Phillips 5级分类法评估大鼠腹腔粘连程度。取粘连盲肠组织切片行HE染色观察粘连组织病理变化,免疫组织化学法观察粘连组织TGF-β1表达变化情况。结果卡巴胆碱干预组腹腔粘连组织面积明显减少,粘连程度评分(0.874±0.641)明显低于腹腔粘连组(3.125±0.641),P<0.01;卡巴胆碱干预组与腹腔粘连组比较,粘连组织HE染色显示炎症反应轻,偶见增生纤维结缔组织;免疫组织化学染色示假手术组与卡巴胆碱干预组TGF-β1表达水平均显著低于腹腔粘连组。结论卡巴胆碱可能通过抑制大鼠腹腔粘连组织炎症反应和TGF-β1表达影响腹腔粘连的形成。[Objective] To investigate the effects of carbachol on postoperative intra-abdominal adhesion formation and expression of transforming growth factor betal (TGF-β1) in adhesion tissues: [Methods] Twenty-two Wistar rats were randomly assigned into sham operated group (n =6), abdominal adhesion group (n =8) and carbachol treated group (n =8, 50 ug/kg). Animal model of abdominal adhesion was produced by scraping on appendix of cecum and abdominal wall. Fourteen days after surgery, rats were separately killed and the adhesion degree was evaluated by Phillips scale. The adhesive tissues were taken to observe histopathological changes and the changes of TGF-β1 were detected by immunohistochemistry. [Results] The scores of intra-abdominal adhesion tissues were significantly lower in the carbachol treated group than that in adhesion group [(0.874±0.641) V.S. (3.125±0.641), P 〈0.01]. Mild inflammatory changes and less fibrous proliferation were observed in carbachol treated group microscopically. Expression of TGF-β1 in the carbachol treated group and sham operated group by immunohistochemistry staining were much lower than that in abdominal adhesion group. [ Conclusion] The carbachol have a significant prevention effect on postoperative abdominal adhesion by inhibiting the expression of TGF-β1 and inflammatory responses in adhesion formation.
