摘 要:目的:观察模型兔膝骨性关节炎软骨中MMP-1、MMP-13及TIMP-1 mRNA表达水平及"三联疗法"对它的影响。方法:将56只大耳白兔随机取8只作正常对照组,48只进行手术方法制作兔膝OA模型。8周后待兔膝骨性关节炎模型建立后,随机将48只造模动物分为模型组、中药薰洗组、功能锻炼组、手法弹拨组、扶他林组、三联疗法组。各治疗组分别进行4周治疗后,取股骨内髁内侧退变软骨用反转录-多聚酶链反应的方法检测各治疗组对软骨中MMP-1-、13及TIMP-1 mRNA表达的影响。结果:正常组软骨中MMP-1、MMP-13及TIMP-1 mRNA表达较低,模型组软骨中MMP-1、MMP-13的mRNA表达明显增高,TIMP-1的mRNA检测为阴性表达。各治疗组软骨中MMP-1、MMP-13的mRNA表达降低,三联疗法组比其它治疗组降低显著(P<0.05),各治疗组软骨中TIMP-1的mRNA表达极低,且各治疗组比较差异无统计学意义。结论:各治疗组均能抑制软骨中MMP-1-、13的mRNA表达,不同程度减轻兔膝骨性关节炎软骨的退变,各治疗组对软骨中TIMP-1的mRNA表达无影响。三联疗法组与其它治疗组在抑制MMP-1、MMP-13在兔膝骨性关节炎软骨中比较差异有统计学意义(P<0.05),说明"三联疗法组"在治疗兔膝骨性关节炎抑制基质胶原溶解破坏减轻软骨退变优于其它治疗组,它对兔膝骨性关节炎的治疗作用机制之一可能与抑制MMP-1-、13表达有关。Objective:To investigate the effect of the three united remedy (passive functional exercise and herb fumigating combining massage) on the expressions of MMPs and TIMP-1 in knee cartilage of osteoarthritis (OA) rabbit. Methods: Eight rabbits were selected randomly from 56 rabbits as control group, and the rest 48 rabbits were made into knee OA models. The model rabbits were then divided into 6 groups: model group, fumigating group, passive functional exercise group, massaging group, Votalin group and three united remedy group. After 4 weeks' treatment, the degenerated cartilages of the rabbits' articular genu were taken and the expressions of MMP-1, MMP-13 and TIMP-1 were measured by RTPCR. Results:In normal control group, the mRNA expression rates of MMP-1, MMP-13 and were relatively low. The mRNA expression of MMP-1 and MMP-13 significantly increased in model group while TIMP-1 mRNA could not be measured. The expression rates of MMP-1 and MMP-13 remarkably decreased in the 4 treatment groups, and in the three united remedy group, the expressions decreased more significantly (P〈0.05). The level of TIMP-1 mRNA was extremely low in all treatment groups, and there was no significant difference between each group. Conclusion:Each treatment could inhibit the mRNA expression rates of MMP-1 and MMP-13 in knee cartilage and reduce the cartilage degeneration in different degrees. But they had no effect on TIMP-1 mRNA expression. The united remedy could inhibit MMP-1 and MMP-13 more efficiently than the rest treatments did, illustrating that the three united remedy functions better in inhibiting decomposition of cartilage ECM collagen and reducing the cartilage degeneration than other treatments. One of the mechanisms might be through inhibition of MMP-1 and MMP-13.
