摘  要:目的分析长春地区部分医院临床分离的肠杆菌科细菌对抗生素的耐药性及超广谱β-内酰胺酶的检出情况。方法采用肠杆菌科细菌生化鉴定管和编码手册鉴定细菌;通过二倍稀释法进行药物敏感试验;利用三代头孢菌素和棒酸纸片扩散法对其进行超广谱β-内酰胺酶检测。结果从临床标本中鉴定出4种肠杆菌科细菌,其中大肠埃希菌占53.4%;肺炎克雷伯菌占25.2%;粘质沙雷菌占12.9%;阴沟杆菌占8.5%。四种肠杆菌科细菌对青霉素钠,氨苄西林钠和头孢拉定的耐药率分别为51.2%-67.1%,72.4%-80.5%和48.8%-57.5%,对头孢曲松、头孢哌酮和头孢噻肟的耐药率分别是26.8%-48.1%、47.6%-59.8%和40.2%-58.8%。超广谱β-内酰胺酶在大肠埃希菌和肺炎克雷伯菌中的检出率较高,分别为29.7%和20.4%。结论在所鉴定的四种肠杆菌科细菌中对临床常用的抗生素有不同程度的耐药性。Objective To investigate drug-resistance and extended-spectrum β-lactamase (ESBLs) of the Enterobacteriaceae d clinical isolates from hospital in Changchun.Methods The strains of Enterobacteriaceae were identified by biochemistry and conventional procedure,drug resistance testing was carried out with double-dilution method. ESBLs was detected with slip diffusion method. Results Four genera in the family of Enterobocteriaceae were found from clinical isolates,they are Escherichiat(53.4%),Klebsiella siella pnapneumoniae (25.2%),Serrat/a marcescens (12.9) and Enterobaaer cloacae (8.5%),respectively.51.2%- 67.1% of these bacterial strains were resistant to penicillin sodium,72.4% -80.5% to ampicillin sodium,48.8% -57.5% to eefradine,26.8% -48.1% to ceftriaxone,47.6% - 59.8% to eefoperazone and 40.2% - 58.8% to eefotaxime. A high rate of ESBI.s detection were found in strains of Escheffr.hm coli and Klebsiella pneumoniae. Condu.sion These data showed that Enterobacteriaceae spices exhibit a relative hgih rate of resistance to antibiotics,it seems very important to monitor susceptibility of Enterobacteriaceae to antibiotics.
