国产阿立哌唑与氯氮平治疗精神分裂症的疗效及安全性对照研究

摘　　要：目的评价阿立哌唑治疗精神分裂症的疗效及安全性。方法82例精神分裂症患者随机分为阿立哌唑组和氯氮平组,观察8周。于治疗前及第2、4、8周末,采用阳性和阴性症状量表(PANSS)评定疗效,采用副反应量表(TESS)评定不良反应。最终纳入分析的79例(阿立哌唑组40例,氯氮平组39例)。结果治疗第8周末,阿立哌唑组PANSS总分(51.75±8.56)分与治疗前比较,差异具统计学意义;总体疗效与氯氮平相当,氯氮平起效较快,4周末PANSS总分与阿立哌唑比较,差异有统计学意义(P=0.008),8周末总分差异无统计学意义(P=0.065)。8周观察期与氯氮平比较,阿立哌唑对心电图、肝功能影响较小,少有体重增加,服药依从性好。结论阿立哌唑对精神分裂症有良好疗效,安全性较好。[ Objective ] To evaluate the clinical efficacy and safety of aripiprazole and clozapine in treatment of schizophrenia. [ Methods] 82 schizophrenia patients randomly divided into aripiprazole group and clozapine group, observation for 8 weeks. PANSS and TESS were applied to evaluate efficacy and advese reactions before treatment and at No. 2, 4, 8 weekends. 79 included in final analysis （aripiprazole group 40 cases, clozapine group 39 cases） [ Results ] After 8-week-treatment, the total scores of PANSS （ 51.75 ± 8.56 ） were significantly decreased （ P = 0. 008 ） in aripiprazole group ; the efficacy of aripiprazole was similar with clozapine, but elozapine effeeted faster. The total scores of PANSS in clozaping group, compared with aripiprazole, the difference was significant （ P = 0.008 ） after 4 weeks, and not significant after 8 weeks （ P = 0.065 ）. Compared with clozapine, aripiprazole had less effect on electrocardiogram and liver function, little weight gain, and good medication compliance. [ Conclusion] Aripiprazole has a good efficacy and better safety for schizophrenia.
