跨上矢状窦双侧硬膜外血肿的急救处理

摘　　要：目的探讨跨上矢状窦双侧硬膜外血肿的病情变化和急救策略。方法回顾分析2005年1月～2006年12月我院收治的10例颅脑创伤及择期开颅手术后发生的跨上矢状窦双侧硬膜外血肿病例,总结分析其急救治疗体会。结果10例跨上矢状窦双侧硬膜外血肿病人通过严密观察分析病情,符合指征而手术9例;血肿较小保守治疗1例。本组病人住院7d～3个月,随访3～12个月,病人住院期间死亡1例,植物生存1例;余8例预后良好。结论跨上矢状窦双侧硬膜外血肿临床少见,病人病情恶化往往较为迅速,需及时发现并处理。Objective To discuss the emergency treatment of acute bilateral epidural hematomas （BEHs） striding over superior sagittal sinus （SSS）. Methods A retrospective analysis was done in 10 cases of EDHs striding over SSS hospitalized between January 2005 and December 2006. Results Nine patients were operated and one was treated nonsurgically. Their hospital stay was seven days to three months. The follow-up was three to 12 months. Eight patients recovered well, one became a plant man and one died. Conclusion Bilateral epidural hematomas striding over SSS are rare emergency, the patients＇ conditions may deteriorate rapidly which need early detection and in-time management.
