乳腺癌患者术前彩超探查腋动脉和胸外侧动脉阻力指数的研究

摘　　要：目的探讨二维超声扫查及多普勒血流动力学在乳腺良、恶性肿块诊断中的价值。方法术前1周对30例乳腺良性肿块患者、30例乳腺恶性肿块患者及25例健康女性进行彩色多普勒超声检查，测量并记录肿块大小、部位、彩色血流分级及胸外侧动脉和腋动脉阻力指数（RI）。结果乳腺癌组肿块大小2～6cm，腋动脉、胸外侧动脉RI与良性组和对照组相比明显降低（P〈0．05或P〈0．01）。结论应用彩色多普勒超声测定胸外侧动脉和腋动脉的RI，可作为诊断乳腺癌的指标之一。Objective To investigate the differential diagnostic value of hemodynamies by 2D ultrasonography and Doppler sonography in benign or malignant breast tumors. Methods 30 patients with benign breast tumor, 30 patients with malignant breast tumor underwent color Doppler sonography 1 week prior to surgical operation, and the sizes and sites of tumor masses, color gradings of blood flow and hemodynamie parameters of the lateral thoracic artery （LTA） and axillary artery （AA） were measured and recorded, with 25 healthy women as the control group. Results In the 30 canc- er patients, the sizes of tumor mass ranged from 2 to 6 cm; the mean axillary artery resistance index （RI） range was 0.81 ± 0.03, and the lateral thoracic artery RI range was 0.75 ± 0.08. In the benign group, the axillary artery RI range was 0.83 ± 0.02 and the lateral thoracic artery RI range was 0.78 ± 0.05 ; while in the control group, the mean lateral thoracic artery RI range was 0.83 ± 0.04 and the axillary artery RI range was 0.83 ± 0.05. Conclusion Detection of the RI of LTA and AA by color Doppler sonography can be used as one of diagnostic indicators of breast cancer.
