摘 要:[目的]查找院前急救各环节存在的问题,降低急救死亡率和伤残率,为建设和完善急救网络提供客观依据。[方法]对照全市2002~2004年度67个急救网络医院经电话呼救院外死亡的病历,填写调查表,对相关数据进行描进性统计分析。[结果]3个年度全市院外救活患者数分别为47341、57488和72833人次,死亡率分别为2.37%、2.05%和2.35%;3年中交通事故在院外死因谱中由第1位下降到第3位,循环系统疾患由第3位上升到了第1位,中毒连续3年保持在第4位;3个年度院外死亡患者求救电话均以:0.00~8:00最多,救护车1 min内出动构成比分别为29.57%、43.05%和73.82%.5 min内到达现场构成比分别为43.29%、25.75%和25.61%;到达现场已死亡者分别占院前死亡的71.56%、78.97%和87.43%。,[结论]2002~2004年度深圳市院外急救人次呈增加之势,死亡率较国内一些城市为低.院外死因谱有所改变.1 min内救护车出动构成比上升,5 min内到达现场构成比下降,院外急救操作实施率提高。[Objective] To find out the problems in out-of-hospital emergency care, and to decrease the mortality anti disabilit? rate of first aid so as to provide objective basis for establishing the emergency network and its improvement. [ Methods] We collected the medical records of death cases from the 67 first aid telephone from 2002 to 2004. Related questionnaires were filled and related data were analyzed with the descriptive analysis. [Results] 47341, 57488 and 72833 patients accepted treatment outside hospital respectively from 2002 to 2004, with the mortality were 2.37%, 2.02% and 2.35%, respectively. The mortality of traffic accident dropped from the first reason of death outside hospital in 2002 to the third in 2004, and the circulation system disease rose from the third reason of death outside hospital to the first, and the intoxation remained the fourth reason of death outside hospital. Most emergency calls for first aid were during 0:00-8:00. The proportions of ambulance reaction in 1 minute during 2002 to 2004 were 29.57%, 43.05% and 73.82%, respectively, and proportions of ambulance reaction in 5 minutes were 43.29%, 25.75% and 25.61%, respectively, what's more, when the staffs an'ived, the death rates of patients were 71.56%, 78.97% and 87.43% respectively during 2002 to 2004. [Conclusion] The number of first aid outside hospital increased from 2002 to 2004. However, the mortality during the first aid outside hospital in Shenzhen was higher than that in other cities. The death reasons did not significant change. The proportion of ambulance reaction in 1 minute had increased, and proportion of ambulance reaction in 5 minutes decreased, in a word, the efficiency of first aid outside hospital has been increased.
