关于建立契约型新型双向转诊模式的初步研究

摘　　要：[目的]了解目前珠海市双向转诊基本情况,探索建立契约式双向转诊新模式,为我市实施"小病治疗免费"项目和全民基本卫生保健服务运行作基础研究,配合建立健全全民基本医疗卫生保健制度和体系,大力发展和推进基层卫生医疗服务,实行双向转诊找出问题和解决方法。[方法]采用统一设计的问卷调查法对珠海市双向转诊现状进行调查,并对数据进行归类分析;运用文献复习法总结提练。[结果]定向契约型双向转诊模式发挥了一定的作用,但同时存在相当多的问题,需要进一步完善相关配套支持政策,比如拉大社区卫生服务机构和综合医院的医保报销标准,建立紧密的信息管理制度,合理调整病人和医疗机构三方的利益关系,建立合理的补偿机制等。[结论]建立契约型新型双向转诊模式符合当前医疗卫生现状的双向转诊模式。[Objective] To study the situation of two-way transferring consultation in Zhuhai. and explore a new type ot contractual dual referral, so as to provide free treatment for indisposition program and basic results for national basic health care service, establish the sound national healih care system, develop and promote the primary health care service, and find out the problem and its solution for performing dual referral. [ Methods] Uniformed questionnaires were used to investigate the era＇rent status of the dual referral pattern, and clustered and analyzed the data. Literature review method was used in the study. [Results] Ahhough the oriented contractual dual referral pattern is effective, a lot of problems are existed, related supporting policies should be further perfected, such as expanding the reimbursement proportion, establishing closely inforrnation management system, reasonable adjusting the profit relationship among the patients, hospital and social medical insurance, and establishing a raJional compensation mechanism [Conclusion] The contractual dual referral pattern is suitable for the cunent medical system.
