汕头地区健康献血者人巨细胞病毒抗体及DNA检测结果的评价

摘　　要：目的:了解汕头地区健康献血者人巨细胞病毒(HCMV)感染情况,并探索一种比较可靠的检测方法。方法:采用酶联免疫吸附试验检测健康献血者血清(450份)中HCMV-IgM和-IgG抗体,并对12份疑似HCMV感染(HCMV-IgM、HCMV-IgG双阳性)、278份HCMV-IgM阴性而HCMV-IgG阳性及10份HCMV-IgM、HCMV-IgG双阴性的血浆样本进行荧光定量PCR(FQ-PCR)检测,并对两种检测方法的结果进行比较分析。结果:汕头地区健康献血者HCMV-IgM、HCMV-IgG阳性率分别为2.7%和93.8%;FQ-PCR对HCMV-DNA的检出率高于HCMV-IgM阳性率(P<0.05)。结论:汕头地区健康献血者HCMV阳性率与国内其他地区学者所报道相近,FQ-PCR定量检测血清HCMV-DNA的方法能够更灵敏地反映HCMV活动性感染,FQ-PCR对献血者HCMV感染的诊断有重要作用。Objective： To understand the status of human cytomegalovirus（HCMV）infection in healthy blood donors in Shantou, and to quest a reliable detection method. Methods： Antibodies IgM and IgG against HCMV（HCMV-IgM and HCMV-IgG）in 450 blood donors＇ sera were tested by enzyme-linked immunosorbent assay（ELISA）. All 12 seropositive donors（IgM-antibody-positive and IgG-antibody-positive）, 278 IgM-antibody-negative and IgG-antibody-positive donors, 10 IgM-antibody-negative and IgG-antibody-negative donors were detected by fluorescent quantitative polymerase chain reaction（FQ-PCR）. The correlation between two methods was analyzed. Results： The positive rate of antibodies lgM and IgG against HCMV was 2.7 % and 93.8 % in serum, the HCMV positive rates of the plasma examined with the method of FQ-PCR were higher than positive rate of HCMV-IgM examined with the method of ELISA（ P 〈 0.05）. Conclusion： The positive rate of HCMV-IgM in healthy blood donors in Shantou is consistent with other researehers＇reports in China. FQ-PCR can sensitively discover active HCMV infection with low detecting HCMV false-negative rate and is absolutely necessary methods for diagnosis HCMV infection in blood donors.
