摘  要:背景:前期实验关于针刀疗法对第三腰椎横突综合征动物模型的治疗机制研究主是从调控软组织炎性反应为主要切入点,但尚未从软组织损伤修复的角度加以研究。目的:观察针刀疗法对第三腰椎横突综合征模型大鼠组织中成纤维细胞增殖的影响。设计、时间及地点:随机对照动物实验,于2008-04/05在解放军海军总医院海战伤研究中心实验室完成。材料:健康雄性SD大鼠68只,其中51只用于制备第三腰椎横突综合征模型。方法:68只SD大鼠按照随机数字表法分为4组,每组17只。针刀干预组:于造模后15d,在麻醉下,用针刀于大鼠第三腰椎横突后半段表皮投影处刺入至深筋膜下,行疏通切割两三刀,出针,按压针孔止血;封闭对照组:在无菌条件下,用0.45mm×12mm一次性无菌注射针刺入腰3横突后半段深筋膜下,注入20g/L利多卡因注射液0.05mL+地塞米松注射液0.01mL+维生素B120.01mL+氯化钠注射液0.13mL,出针,按压止血。模型对照组造模但不干预、正常对照组不给予任何干预措施。主要观察指标:于造模后15d各组取1只大鼠行组织形态学观察;造模后21,30d,每组各处死8只大鼠,取材后,行苏木精-伊红染色,免疫组织化学检测增殖细胞核抗原的表达,并计算成纤维细胞的增殖指数。结果:68只SD大鼠均进入结果分析。①造模后15d正常对照组结构正常;其他3组血管充血,并见淋巴小结形成。造模后21d针刀干预组与封闭对照组大鼠组织内成纤维细胞较少,毛细血管增生扩张,胶原重建开始;模型对照组成纤维细胞较多,胶原分泌增多。造模后30d模型对照组成纤维细胞增生,可见红细胞散在分布,有增生的毛细血管;针刀干预组与封闭对照组可见纤维组织排列成行;针刀干预组较其他各组胶原纤维形成减少,瘢痕面积小。②造模后21d正常对照组可偶见增殖的成纤维细胞;封闭对照组与针刀干预组大鼠组织内的成�BACKGROUND: Early experiments regarding the acupotomology therapy on the third lumbar transverse process syndrome animal model of the treatment mechanism take the regulation of soft tissue inflammatory response as main entry point; however, the perspective of soft tissue healing is few. OBJECTIVE: To observe the effect of acupotomology intervened third lumbar transverse process syndrome rat on levels of the fibroblast proliferation. DESIGN, TIME AND SETTING: A randomized controlled animal experiments was performed at the Naval Injury Laboratory Research Center of Navy General Hospital from April to May 2008. MATERIALS: Sixty-eight Sprague Dawley rats, 51 of which were prepared for the third lumbar transverse process syndrome models. METHODS: Sixty-eight rats were divided into 4 groups with the random number table, with 17 rats in each group. Acupotomology intervention group: after 15 days, under anesthesia, with acupotomology in the third lumbar transverse process in rats after the second half of epidermal projection department piercing deep fascia, cut 2 or 3 knives, and compressed hemostasis pinhole. The blockade control group: under sterile condition, 0.45 mm × 12 mm disposable sterile injection needle was injected into in the third lumbar transverse process in rats after the second half of deep fascia, and 20 g/L lidocaine injection 0.05 mL plus dexamethasone injection 0.01 mL plus Vitamin B12 0.01 mL plus sodium chloride injection 0.13 mL was injected. There were no other intervention in the model control group and normal control group. MAIN OUTCOME MEASURES: At 15 days after model preparation, one rat was obtained from each group to underdone morphological observation. At 21 and 30 days after model preparation, 8 rats were killed in each group, hematoxylin-eosin staining and immunohistochemical method were used to detect the expression of the proliferating cell nuclear antigen, and to calculate fibroblast proliferation index.
