摘 要:目的进一步阐明病毒在肾综合征出血热肾脏损害中的作用。方法应用套式-逆转录-聚合酶链反应对31例肾综合征出血热患者79份尿液沉渣细胞中病毒核酸进行检测。结果79份样本中58份阳性,阳性率为73.4%在病程早期(1~7天)各型患者的检出率为100%;在病程后期(≥13天),轻、中、重型患者的检出率分别为0%、16%和85%,重型患者的检出率较中型、轻型患者高(P<0.01,P<0.05);且检出率与肾功能损害密切相关。结论早期存在病毒对肾脏有直接损伤作用,在重型患者病毒的直接作用为肾脏损害的主要因素。Objective In order to explore direct cytopathic effect of HFRS virus to kidney in the patients with hemorrhagic fever with renal syndrome (HFRS). Methods We detected HFRSV RNA in urinary sediments of 79 samples from 31 HFRS patients by nested reverse transcription polymerase chain reaction (nested RT PCR). Results HFRSV RNA in 58 of 79 (73.4%) samples were posive. During the early stage of HFRS (1 ̄7days), HFRSV RNA were detected in all samples (100%). During the late stage of HFRS(≥13days), the posive ratio of HFRSV RNA in mild moderate and serious patients were 0%, 16% and 85% respectively. The HFRSV RNA were detected more often in the serious patients with HFRS than that in the mild and moderate patients ( P<0.01,P <0.05).The detection rate of HFRSV RNA was correlated with the level of renal impairment. Conclusion It suggested that there is direct cytopathic effect of HFRSV virus on kidney, which seems to be the major pathogenic factor in serious patient and the impairment of renal functions may occur in the early stage.
