摘　　要：为了研究血浆各种脂质水平与冠状动脉硬化程度的关系,将132例病人按冠脉造影结果分为冠心病组(狭窄≥50％)83例,与非冠心病组(狭窄<50％)49例,并按冠状动脉狭窄程度分级,将各支冠脉级别累加为冠脉狭窄程度记分(CSS)。同时检测各项血脂指标[Lp(a)、TC、TG、HDL-C、LDL-C、apoAI、apoB、TC/HDL-C、apoAI/apoB]。结果表明,患者的年龄、Lp(a)、LDL-C、TG、TC、HDL-C、TC/HDL-C、apoAI/apoB和apoB与非冠心病组比较差异均有显著性。多元相关及逐步回归分析表明,CSS与年龄、Lp(a)、TC/HDL-C、apoB水平呈明显正相关(均P<0.01)。排除年龄因素的影响,血浆Lp(a)、TC/HDL-C及apoB是冠心病的独立危险因素,对于预测冠心病及病变程度较其它血脂指标更为敏感,具有更大的临床价值。To study the relationship between the levels of plasma lipoproteins and severity of coronary atherosclerosis, 132 patients undergoing diagnostic coronary angiography were divided into two groups: 83 patients(≥50% diameter narrowing) were in CHD group,49 patients(<50% diameter narrowing) were in non-CHD group,and graded all the patients according to the coronary stricture score (CSS). Plasma lipoproteins including Lp (a ), TC, TG, HDL-C, LDL-C, TC/HDL-C, apoAI, apoB, apoAI/apoB were simultaneously examined in two groups. The results showed that the age,Lp(a) ,TC,TG,LDL-C,apoB,HDL-C,the ratio of TC/HDL-C and apoAI/apoB had significant differences between the two groups. The multiple stepwise regression analysis showed that distinctly positive correlation of CSS associated with the levels of Lp (a), TC/HDL-C, apoB and age, respectively (all P<0. 01). The results suggested that besides age,the Lp(a),TC/HDL and apoB were the most powerful independent variables associated with the presence and severity of CHD. They were more sensitive indices in predicting the severity of CHD than other indices and they were of great clinical application value.
