摘 要:目的探讨神经内窥镜辅助下半导体激光治疗颅内蛛网膜囊肿的临床应用价值。方法48例颅内蛛网膜囊肿患者均采用神经内窥镜辅助下利用半导体激光,选择适当的输出功率进行凝固、止血、汽化、切割等手术治疗。结果随访48例,随访时间6个月至2年。45例症状消失或明显得到改善,无复发;3例术后症状未改善:并发硬脑膜下血肿1例,蛛网膜下腔出血2例,均经保守治疗痊愈,无感染等其他并发症;总有效45例(93.75%)。结论神经内窥镜配合半导体激光技术治疗颅内蛛网膜囊肿创伤小,术后并发症少,疗效确切,是临床值得推广的治疗方法。Objective To investigate the curative effects of endoscopic neurosurgery combined with semiconductor laser on intracranial arachnoid cyst (IAC). Methods The patients with intracranial arachnoid cyst were treated by adopting simple endoscopic neurosurgery, admoving optical fiber transits porous channel to the diseased region and selecting adequate output power for operative procedure such as coagulate and hematischesis (5 -12W) ,gasification and incision(15 -30W). Results After 6 to 24 months,48 cases have been followed up. Through head CT/MRI dynamic observation, 45 cases with intracerebral cysts get satisfiable operative effectiveness, were improved distinctly, but 3 cases ware no effective. After operation, 1 case of epidural hematoma and 2 cases of subarachnoid hemorrhage supervened, no infection, no cardio-pulmonary function abnormity and other complications. All effective rate is 93.75%. Conclusion It is safe and effective for endoscopic neurosurgery combined with semiconductor laser to treat intracranial arachnoid cyst and has less trauma and less complications.
