摘 要:目的探讨巨大切口疝无张力修补术对腹腔压力和呼吸、循环功能的影响。方法采用膀胱测压法监测86例巨大切口疝无张力修补术后6h和72h腹腔内的压力,同时观察患者的呼吸频率和心率。结果术后72h的腹腔压力、呼吸频率和心率显著高于术后6h,差异具有统计学意义(P〈0.05)。结论巨大切口疝无张力修补术可导致腹腔压力明显升高而引起呼吸和循环功能的障碍。围手术期合理降低腹腔压力有助于降低并发症发生率。Objective To investigate the effect of tension-free repair of large incisional hernia on respiratory and circulatory function. Methods Cystometry was used to determine intra-abdominal pressure (IAP) at 6th and 72nd h after tension -free repair of large ineisional hernia in 86 patients, and simultaneously, heart rate and breathing rate were recorded. Results lAP, heart rate and breathing rate were increased significantly ( P 〈 0.05 ) at 72nd h compared to 6th h after operation. Conclusion Tension-free repair of large incisional hernia contributes to intra-abdominal high pressure and leads to respiratory and circulatory dysfunction. And complications will be decreased when intra-abdominal pressure is decreased reasonably.
