摘 要:目的:探讨术前使用氟比洛芬酯对全身麻醉用药量的影响。方法:75例接受桥小脑角显微神经血管减压手术的患者随机分成3组,每组25例,术中静脉吸入复合维持麻醉。A组未使用氟比洛芬酯。B组麻醉诱导前静脉注射氟比洛芬酯1mg/kg。C组麻醉诱导前静脉注射氟比洛芬酯2mg/kg。手术中吸入笑气的浓度不变,根据熵指数监测与血流动力学指标变化调节静脉麻醉药用量。记录手术时间、血流动力学指标、异丙酚与雷米芬太尼用量。结果:3组患者的一般情况、手术时间相同。A组患者手术中异丙酚的用量为183.49±29.82mg,雷米芬太尼用量为563.45±9.61μg。B组患者手术中异丙酚的用量为171.18±34.82mg,雷米芬太尼的用量为496.44±69.07μg。C组患者异丙酚的用量为143.2±29.03mg,雷米芬太尼的用量为363.60±51.86μg。与A组相比,C组患者手术中异丙酚与雷米芬太尼用量显著减少(P<0.05)。结论:术前静脉注射氟比洛芬酯1mg/kg对手术中麻醉药用量影响不大。而静脉注射2mg/kg能明显减少手术中麻醉药用量。Objective:To investigate the effect of pre-operative flurbiprofen on anesthetics dosage during general anesthesia.Methods:Seventy-five patients were randomly allocated to 3 groups:Group A(n=25):no pre-operative flurbiprofen.Group B(n=25):1mg/kg flurbiprofen i.v.before anesthesia induction.Group C(n=25):2mg/kg flurbiprofen i.v.before anesthesia induction.Anesthesia induced with propofol and fentanil,maintained with propofol-remifentanil infusion combined with nitrous-oxide inhalation.Nitrous-oxide inhalation concentration was 50%,propofol and remifentanil dosage adjusted according to the hemodynamic parameter and entropy monitoring.Anesthesia time,anesthestics usage and hemodynamic data were recorded.Results:No significant differences were found in age,body mass index and operation duration among the three groups.The mean dosage of propofol and remifentanyl in group A was 183.49±29.82mg and 563.45±9.61μg respectively.The mean dosage of propofol and remifentanyl in group B was 171.18 ±34.82mg and 496.44 ±69.07μg respectively.The mean dosage of propofol and remifentanyl in group C was 143.2±29.03mg and 363.60±51.86μg respectively.Compared with group A,propofol and remifentanyl dosage in group C was obviously reduced(P〈0.05).Conclusion:Pre-operative flurbiprofen 1mg/kg does not affect the anesthetic dosage,but pre-operative flurbiprofen 2mg/kg can reduce the anesthetics dosage.
