摘　　要：目的评价自我管理对社区老年高血压患者生活质量的影响及其血压控制效果。方法调查对象为纳入社区三级管理的227例老年原发性高血压患者，随机分为干预组（自我管理＋三级管理）123例和对照组（三级管理）104例。应用健康状况问卷（SF36）调查生活质量，并检测两组的动态血压指标。结果与对照组相比，干预组干预后的血压指标，SF-36量表的总分及生理职能、一般健康状况、精力、社会功能、情感职能及精神健康因子差异有统计学意义（P〈0．01或0．05），生理机能及躯体疼痛因子差异无统计学意义（P〉0．05）。结论社区自我管理在降低老年高血压患者血压水平及提高生活质量方面有积极作用。Objective To evaluate the influence of hypertension self-management on the quality of life and the blood pressure control of the elderly hypertension patients in the community. Method A community based randomized controlled trial was conducted among 227 elderly patients with hypertension recruited by voluntary registration, who were randomized into 2 groups, intervention group （n= 123） attended self-management, and control group （n= 104） receiving routine hypertension management. Baseline and 6-month follow up assessment were conducted to analyze the quality of life indicators and blood pressure indicators of the two groups measured. Results After the intervention, blood pressure of the intervention group was significantly lower compared with the control group （P〈0.01）. The intervention patient＇s total score of SF was significantly higher than that of control group and the interyention patient＇ s scores of 6 factors of SF-36, including Role-Physical, General Health, Vitality, Social Functioning, Role-Emotional and Mental Health, were significantly higher than those of the control group （P〈0.01 or 0.05）. Conclusion Community self management in elderly hypertension patients can significantly reduce their blood pressure levels and enhance the quality of life.
