超声心动图评价缬沙坦对高血压病左室重构的逆转作用

摘　　要：目的应用超声心动图技术评价缬沙坦对原发性高血压病左室重构的影响。方法原发性高血压痛患者178例应用缬沙坦80mg／d口服治疗，疗程12个月。观察治疗过程中患者血压及症状体征的变化情况，并分别在治疗前及治疗后每3个月分阶段应用超声心动图观察心脏形态学变化情况。结果缬沙坦在治疗二周后血压开始下降，五周后趋于平稳；心腔径、室壁厚度及左室质量指数等在治疗3月后开始降低（P〈0．05），6个月～1年后明显下降（P〈0．01）并趋于稳定。结论缬沙坦能够有效逆转原发性高血压病所导致的左室重构，超声心动图能够全面、客观地定量评价心脏结构、室壁运动状态。Objective To assess the effect of valsartan on left ventficular reconstitution in patients with essential hypertension by echocardiography. Methods Oral administration of valsartan, 80 mga day, was made to 178 cases with essential hypertension and the course lasted for 12 months; blood pressures and the changes of symptoms and signs in the treatment were observed, and echocardiography was applied in observing the morphological changes of the heart before treatment and every 3 months after treatment. Results 2 weeks after the treatment with valsartan, the blood pressures of the patients started decreasing and changed to he steady 5 weeks after treatment; the cardiac cavity diameter, ventricular wall thickness and left ventrieular mass index began to decrease 3 months after treatment （ P 〈0. 05 ）, and from 6 months to 1 year after treatment, they decreased obviously （ P 〈0. 01 ） and tended to be stable. Results Valsartsn can effectively reverse the left ventricular reconstitution in patients with essential hypertension; echecardiography can objectively make an overall quantitative analysis on cardiac structure and ventrieular wall motion state.
