摘  要:目的探讨尿路感染产超广谱β-内酰胺酶(ESBLs)菌株的耐药性及其危险因素。方法选取尿培养结果为革兰阴性杆菌的患者168例,其中产ESBLs菌尿路感染患者63例为观察组,非产ESBLs菌尿路感染患者105例为对照组,对两组尿路感染细菌的耐药性进行比较,并采用Logistic回归分析产ESBLs菌株致尿路感染的危险因素。结果产ESBLs组对头孢菌素等多种抗菌药物的耐药性显著高于非产ESBLs组,差异有统计学意义(均P<0.01);第3代头孢菌素的使用和留置尿管等是产ESBLs菌致尿路感染的独立危险因素(相关危险度值分别为15.121和8.847)。结论产ESBLs菌株耐药性严重,临床上应采取有效措施加以防控。Objective To explore antimicrobial resistance and risk factors of extended spectrum β-lactamases (ESBLs)-producing strains in urinary tract infection (UTI). Methods Totally 168 patients who were urine gram negative bacilli positive were individed into observation group (n = 63, ESBLs-producing group) and control group (n = 105,ESBLs non-producing group). The drug resistant spectrum was compared between two groups, while logistic regression analysis was adopted to investigate the clinical risk factors of UTI caused by ESBLs-producing strains. Results The resistant rates of ESBLs-produeing strains to some antibiotics were significant higher than those of non- ESBLs-producing ones (P〈0.01). Use of the third generation cephalosporins and detaining of urinary canal were the independent risk factors of UTI caused by ESBLs-producing strains (OR = 15. 121,OR= 8. 847). Conclusion The state of antimicrobial resistance of ESBLs-producing strains is serious. Some measures should be taken to prevent and control the spread and transmission of ESBLs-producing strains.
