慢性心力衰竭患者血清甲状腺激素和皮质醇改变与心功能的相关性

摘　　要：目的观察慢性心力衰竭(CHF)患者血清甲状腺激素(TH)及皮质醇(F)的水平改变及其与心功能的相关性。方法采用化学发光免疫法,测定51例CHF患者血清TH及F水平,并测定其左室射血分数(LVEF),与20例健康者对照分析。结果CHF组血清TT3、TT4较对照组(NC)下降(P<0.05),rT3较NC组升高(P<0.05),且CHF组随着心力衰竭的加重,TT3、TT4水平逐渐减低,rT3水平逐渐升高;FT3、FT4、TSH及F水平在CHF各组与NC组比较无统计学意义(P>0.05)。结论CHF患者常伴有正常甲状腺病态综合征。血清TT3、TT4及rT水平的变化可作为评价CHF患者病情严重程度的客观指标。Objective To observe the changes of thyroid, cortisol hormone and cardiac function in patients with chronic congestive heart failure. Methods Serum TT3 ,TT4 ,FT3 ,FF4 ,rT3 ,TSH and F were measured by chemiluminescence enzYme immunoassay in observation group and control group. Results Comparing with that in control group, TT3,TT4 was decreased significantly in observation group （P 〈 0.05 ）. rT3 was increased significantly in observation group compared with that in control group （ P 〈 0. 05 ）. And patients whose heart functions were worse, the levels of TT3,TT4 were lower and the levels of rT3 were higher. Conclusion Thyroid hormone of patients with CHF was abnormal. The changes of TT3,TT4, and rT3 could be the index for evaluating the state of the illness of patients with CHF.
