依达拉奉与乙酰谷酰胺联合治疗重度颅脑损伤

摘　　要：目的观察依达拉奉与乙酰谷酰胺联合治疗重度颅脑损伤的临床疗效。方法将80例重度颅脑损伤患者随机分为治疗组(n=40)和对照组(n=40),两组患者入院后常规脱水、呼吸机辅助、护脑、促醒、抗感染和营养支持,有手术指征者行去骨板减压、脑室引流及血肿清除术。治疗组给予依达拉奉30mg加入生理盐水100mL,iv,gtt,bid×14d;乙酰谷酰胺0.75加入5%葡萄糖注射液250mL,iv,gtt,qd×14d。观察两组患者的清醒人数、觉醒时间、GCS评分的变化,并分析两组的临床疗效。结果治疗组清醒人数所占比例及治疗后GCS评分均高于对照组(P<0.05),觉醒平均时间较对照组缩短(P<0.05),治愈率及总有效率高于对照组(P<0.01)。结论依达拉奉与乙酰谷酰胺联合使用除能有效地清除自由基、缓解脑血管痉挛外,能更好地营养缺血损伤区的脑细胞,减少凋亡,使患者提前苏醒,对重型颅脑损伤具有明显疗效,值得推广。Objective To evaluate clinical effect about edaravone and aceglutamide injection on severe cerebral trauma. Methods A total of 80 patients with acute cerebral trauma were given randomly to divide into the treatment group （n=40） and the control group （n=40）, After two group of patients are hospitalized the convention to dehydration, to assist breathe by ventilator, protect cerebrum function, accelerate revive, anti-infection and nutrition, if has the surgery to open the skull for reduce pressure in cranium, to drainage the ventricles of the brain and to eliminate hematoma in cranium and so on.The treatment group were given edaravone injection 30rag was dissolved in normal saline 100ml, iv, gtt, bid×14d; and aceglutamide injection.0.75 was dissolved in 5% glucose 250ml, iv, gtt, qd×14d; To observes all patient＇s sober populations, the awakening time, the GCS grading, and analyzes twO groups of clinical effects. Results After the GCS grading and sober population accounts for the proportion in the treatment group is higher than the copatrol group （P〈0.05） ,Awakens the mean time to compare the obvious reduction （P〈0.05）, The cure rate and the total effectiveness are higher than the control group obviously （P〈0.01）. Conclusion According to reaches which edaravone and aceglutamide injection together on severe cerebral trauma, that could to nourishing brain cell ,to reduce cerebrocyte apoptosis in ischemic and injure area and earlier awakening so on .besides eliminate free radical, to alleviate spasm of brain vessel,There are obvious effects in treatment of severe cerebral trauma, therefore it＇s worth to wide application.
