摘  要:目的:探讨彩色多普勒超声心动图诊断新生儿非青紫型先天性心脏病的临床价值。方法:2005年5月~2007年1月本院出生活产儿9612例,其中临床疑诊为先心病225例行彩色多普勒超声心动图检查,可疑或明确诊断病例出院后继续定期随访观察。结果:发现非青紫型135例,其中室间隔缺损43例(31.9%),继发孔型房间隔缺损54例(18例明确为房间隔缺损,36例为房间隔缺损可疑),动脉导管未闭35例(25.9%)。38例室间隔缺损患儿随访发现缺损自闭及变小12例,无明显变化16例,扩大10例,15例明确为房间隔缺损患儿随访发现缺损自闭或变小2例,31例房间隔缺损可疑患儿随访发现自闭或变小26例。29例动脉导管未闭患儿在3个月内随访发现自闭或变小22例。在随访后确诊为先心病患儿共69例,检出率为7.18‰。结论:彩色多普勒超声心动图可作为诊断新生儿非青紫型先心病的重要手段。Objective: To explore the clinical value of color dopple echocardiography in the diagnosis of non-cyanotic congenital heart disease (CHD) in neonates. Methods: From May 2005 to January 2007, 225 CHD neonates diagnosed by color dopple echocardio- graphy from 9 612 neonates were selected and followed up regularly after discharge. Results: 135 neonates were diagnosed as non -cyanotic CHD, 43 neonates (31.9%) were found with ventricular septal defect (VSD), 54 neonates were found with atrial septal defect (ASD) (in- cluding 18 neonates diagnosed definitely and 36 neonates suspected), 35 neonates (25.9%) were found with patent duetus arteriosus (PDA) . 38 neonates with VSD were followd up, 12 neonates closed spontaneously or got smaller, 16 neonates were unchanged, 10 neo- nates enlarged; 31 neonates with ASD suspected were followd up, 26 neonates closed spontaneously or got smaller, while only 2 neonates a- mong the 15 neonates with ASD were diagnosed definitely. When 29 neonates with PDA were followed up within 3 months after birth, 22 ne- onates closed spontaneously or got smaller. 69 neonates were diagnosed as CHD (7.18‰) definitely after follow -up. Conclusion: Color dopple echocardiography is an important method to diagnose non- cyanotic CHD in newborns.
