摘 要:目的观察高碳酸血症对兔急性肺损伤(ALI)模型是否具有保护作用,并观察其对ALI时一氧化氮(NO)依赖途径的影响,探讨其可能的作用机制。方法22只新西兰大白兔随机分为对照组(C组)、非高CO2通气组(N组)、高CO(28%CO2)通气组(H组)。N组和H组通过静脉注射油酸(0.1ml/kg)复制ALI模型,观察肺组织中诱导型一氧化氮合酶(iNOS)活性、NO水平、硝基酪氨酸的含量变化,监测肺生理指标的变化,测定肺湿干重比、肺通透指数等指标,并观察肺组织病理学改变。结果肺组织中iNOS活性水平、NO水平以及硝基酪氨酸含量H组均明显低于N组(P<0.05)。肺湿/干重比、肺通透指数H组均低于N组(P<0.05)。H组气道峰压显著低于N组,动态肺顺应性显著高于N组,动脉血氧分压H组明显高于N组(均P<0.05)。H组病理组织学改变较N组明显减轻。结论机械通气时吸入8%CO2所致的高碳酸血症对ALI动物模型有一定的保护作用,其机制可能与高碳酸血症抑制了NO依赖途径有关。Objective To investigate whether hypercapnia is protective in the rabbit model of acute lung injury (ALl), and to study tile effect of hypereapnia on nitric oxide ( NO )-dependent pathway and the potential mechanism. Methods Twenty-two New Zealand white rabbits were randomly assigned to control group (group C, n =6 ), normoeapnia group (group N, n =8 ),and hypercapnia group (group H, n = 8). The rabbits in groups N and H were intravenously injected with oleic acid (0.1 ml/kg) to establish the ALl model. The levels of inducible nitric oxide synthase (iNOS), NO, and nitrotyrosine in the lung tissues were detected. The changes in physinlogical parameters were monitored. Lung wet/dry weight ratio and pulmonary permeability index were measured, and the pathological ehages were assessed. Results The levels of iNOS, NO, and nitrotymsine in the lung tissues, the lung wet/dry weight ratio, and the pulmonary permeability index were significantly lower in group H than in group N ( all P 〈 0.05 ). Compared with group N, the peak airway pressure was significantly lower and the dynamic lung compliance and PaO2, significantly higher in group H (P 〈 0.05 ). Histopathological changes in group H were less severe. Conclusion Hypercapnia is protective in the rabbit model of ALl. The mechanism might be associated with the inhibition of NO-dependent pathway by hypercapnia.
