摘  要:目的观察兔行不同切削深度的准分子激光角膜上皮瓣下磨镶术(LASEK)后角膜基质细胞凋亡的变化。方法健康新西兰白兔随机分为3组,随机选择一只眼行LASEK术,分别按近视-3.00、-9.00和-15.00D设计切削,另一只眼为对照眼。24h和7d后常规取角膜制成石蜡切片,采用核苷酸末端转移酶介导的dUTP缺口标记(TUNEL)法检测基质细胞凋亡,计数凋亡细胞,计算凋亡指数进行分析。结果凋亡细胞呈棕黄色,体积缩小,核固缩成团。术后24h-3.00、-9.00和-15.00D三组激光切削眼凋亡指数分别为0.39±0.10、0.55±0.08和0.54±0.12,均分别高于对照眼;-9.00D的切削较-3.00D组凋亡细胞增多,-15.00D组凋亡指数与-9.00D无显著性差异;术后7d激光切削组的凋亡指数与对照眼无显著性差异,且较24h减轻。结论LASEK术可导致兔角膜基质细胞凋亡,且随切削深度的增加,细胞凋亡呈增加趋势。Objective To observe rabbit corneal stromal cells apoptosis after laser-assisted subepithelial keratecto- my (LASEK). Methods Healthy male or female New Zealand white rabbits weighing 3.0 -4. 0 kg each were included in this study. One eye of each rabbit, selected randomly, was subjected to LASEK. Three corrections were performed : - 3.00, - 9.00 and - 15.00 Diopter for myopia. The contralateral eyes served as controls. Paraffin sections of cornea tissue were detected by terminal deoxyribonucleotidyl transferase-mediated deoxynuridine triphosphate nick end labeling (TUNEL). Apoptosis cells of stromal cells were counted under high power of inverted microscope and images were collected. Results Typical TUNEL positive cells with karyopyknosis in cytoplasm were stained by Buffy. 24 h after LASEK, apoptosis index (AI) of corrected eyes in -3.00, -9.00 and -15.00D correction groups (0. 39±0. 10, 0. 55 ±0. 08 and 0. 54 ±0. 12 respectively) were higher than that of their contra- lateral eyes. -15.00D and -9. 00D corrections caused more apoptotic cells than -3.00D correction. By 7 days after operation, differences were not seen between laser-treated eyes and untreated eyes. Conclusion LASEK can elevate apoptosis level of rabbit corneal stromal cells. Deeper artherectomy seemed caused more apoptotic cells.
