联合检测血清中C1q抗体、IL-18对系统性红斑狼疮的诊断价值

摘　　要：目的探讨联合检测血清中C1q抗体、IL-18对狼疮活动和狼疮性肾炎(LN)的价值。方法使用酶联免疫吸附试验(ELISA)对43例LN患者血清进行C1q抗体和IL-18检测,并与28例无肾炎临床表现的SLE患者作对照。结果LN患者血清中IL-18与C1q抗体浓度显著高于对照组(P均<0.05)。LN患者中C1q抗体与IL-18阳性率高于对照组(P均<0.05),并与系统性红斑狼疮疾病活动指数(SLEDAI)呈正相关(r=0.817、0.728,P<0.05)。结论在LN患者中,存在着C1q抗体和IL-18的高表达。血清中C1q抗体和IL-18在LN发病中起重要的作用,是反映SLE患者并发肾脏损害的重要指标,在LN诊断和判定其活动性方面有重要作用。Objective To explore the value of Clq antibody and the serum levels of IL-18 combination detection in system lupus erythematosus（SLE） activity and lupus nephritis（LN）. Methods ELISA assay was performed to examine Clq antibody and interleukin-18 in the sera of 43 patients with LN and 28 patients of systemic lupus erythematosus without renal damage. The standard methods were used for laboratory testing. Results Clq antibody and interleukin-18 were significantly higher in LN than in controls（P〈0.05）. The positive rate of Clq antibody and interleukin-18 were significantly higher in LN than in controls（P〈0.05）,and there were correlated positively with SLEDAI scores（r=0. 817,0. 728,P〈0.05）. Conclusion In patients with LN,the serum levels of Clq antibody and interleukin-18 are significantly higher than control groups. They are suggested to play a role in the pathogenesis of LN. Clq antibody and interleukin-18 can be used as diagnostic tools and disease activity markers.
