不同外科皮瓣修复先天性尿道下裂的临床疗效观察

摘　　要：目的观察不同外科皮瓣修复先天性尿道下裂的临床疗效。方法回顾性分析我院2000年5月-2008年5月采用不同外科皮瓣治疗的30例先天性尿道下裂患者病例资料。结果不同类型尿道下裂修复应用不同外科皮瓣：Ⅰ、Ⅱ、Ⅲ型尿道下裂修复主要应用阴茎阴囊皮瓣＋尿道板、尿道板，Ⅳ型尿道下裂修复主要应用阴茎阴囊皮瓣、阴茎阴囊皮瓣＋尿道板；阴茎阴囊皮瓣＋尿道板对Ⅰ、Ⅱ、Ⅲ型尿道下裂治愈率无明显差异（P〉0．05），尿道板对Ⅲ型尿道下裂的治愈率低于Ⅰ型和Ⅱ型尿道下裂（P〈0．05）：阴茎阴囊皮瓣＋尿道板、尿道板分别对Ⅰ型和Ⅱ型尿道下裂治愈率相似（P〉0．05），但阴茎阴囊皮瓣＋尿道板对Ⅲ型尿道下裂治愈率显著高于尿道板（P〈0．05）。阴茎阴囊皮瓣＋尿道板、尿道板治疗尿道下裂术后尿瘘发生率分别为15．宜的外科皮瓣；阴茎阴囊皮瓣＋尿道板较适用0％和35．0％。阴茎阴囊皮瓣无并发症发生。结论手术治疗不同类型尿道下裂应选择适于各型尿道下裂的修复治疗，单独尿道板较适用于Ⅰ型和Ⅱ型尿道下裂的修复治疗。Objective To observe the different surgical flap congenital hypospadias clinical efficacy. Methods： Retrospective analysis of 2000 years in our hospital in May - May 2008 using different surgical flaps treated 30 cases of congenital hypospadias in patients with clinical data. Results： The different types of hypospadias repair using different surgical flaps： Ⅰ, Ⅱ,Ⅲ hypospadias repair major applications penoscrotal flap ＋ urethral plate, urethral plate, IV hypospadias repair major applications penoscrotal flap, penoscrotal flap ＋ urethral plate; Penoscrotal urethral plate flap ＋ Ⅰ, Ⅱ, Ⅲ hypospadias cure rate was no significant difference （P〉 0.05）, urethral plate type Ⅲ hypospadias cure rate is lower than Ⅰ type and Ⅱ hypospadias cleft （P 〈0.05） ： penis and scrotum flap ＋ urethral plate, urethral plate, respectively, on the Ⅰ-type and Ⅱ hypospadias similar cure rate （P〉 0.05）, but the penis and scrotum flap ＋ urethral plate for hypospadias type Ⅲ The cure rate was significantly higher than split urethral plate （P 〈0.05 ） . Penoscrotal flap ＋ urethral plate, urethral plate after treatment of hypospadias fistula rates were 15.0% and 35.0%. Penoscrotal flap without complications. Conclusion： Surgical treatment of different types of hypospadias should choose appropriate surgical flaps; penoscrotal flap ＋ urethral plate than that applicable to all types of hypospadias repair treatment, separate urethral plate more applicable to Ⅰ-type and Ⅱ hypospadias restoration treatment.
