摘  要:目的:探讨MRI评价及预测胎儿肺发育情况的临床应用价值。方法:60例单活胎儿(胎龄19~39周)分为A组(胸部及羊水量正常)和B组(胸部或羊水量异常),采用单次激发FSE序列(ssFSE)行横轴面、矢状面和冠状面T2WI扫描,在MRI工作站上分别测量胎儿总体积(FBV)、肺体积(TFLV)、肝体积、羊水量及胎儿肺-肝信号强度比(USIR)等,分析和评价胎儿肺发育情况。结果:60例胎儿肺体积为7.45~108.74 cm3,胎儿总体积120~3500 cm3,TFLV与FBV有极显著相关性(R2=0.89,P<0.001),TFLV与孕周(GW)成线形正相关(R2=0.54,P<0.01),随孕周增加而增加,其中B组胎儿肺体积较A组胎儿明显减小(P<0.05)。通过测量60例胎儿不同孕周LLSIR获得的曲线显示胎儿肺-肝信号强度比随着孕周的增加而增大,LLSIR范围约0.79(孕周为19周)~3.57(孕周为39周),其中B组16例胎儿LLSIR明显低于A组(P<0.05),在LLSIR 95%可信区间之外。结论:胎儿肺体积及LLSIR对于评价胎儿肺发育情况有很好的临床应用价值。Objective: To study the role of MRI in the evaluation of fetal lung development. Methods: MR scanning was performed in 60 alive fetus with the gestational age ranged from19 - 39w, which were divided into Group A (normal chest and a normal volume of amniotic fluid) and Group B (abnormal chest or abnormal volume of amniotic fluid). Sirlgle shot FSE sequence (ssFSE) was used to obtain T2-weighted images on sagittal and coronal planes. The total volume of the fetus (FBV),volume of lung (TFLV),liver,amniotic fluid as well as fetal, lung to liver signal intensity ratio (LLSIR) were measured on MR workstation. The condition of fetal lung development was analyzed and evaluated. Results:Of the 60 fetus, the volume of lung (TFI.V) ranged from 7. 454108.74cm^3 ,the FBV ranged from 120-3500cm^3 ,with a positive correlation (r2=0. 89,P〈0. 001). TFLV showed a linear positive correlation with the gestational age (GW) (r2 =0. 54, P〈0. 001),which ascends as the increase of GW,the fetal lung volume of group B is much smaller than that of group A (P〈0. 05). The curve of LLSIR measured through different gestationai age proved that it increased as the growth of gesta tional age. The extent of LLSIR was about 0. 79 (for 19 weeks of gestational age) to 3. 57 (for 39 weeks of gestational age) ,the LLSIR of 16 fetus in group B was obviously lower than that of group A (P〈0.05) ,which placed out of the 95% prediction interval. Conclusion:Total fetal lung volume and LLSIR played an important role in the evaluation of fetal lung development.
