摘 要:目的:探讨高频超声结合血流定量测量评价系统性红斑狼疮(SLE)患者指端微血管病变的临床价值。方法:系统性红斑狼疮患者30例,其中18例患者未治疗、12例为治疗后患者,正常对照组30例,行高颅超声结合血流定量检查观察各组受试者右手中指末节指掌血管分布特征、走行、数量,测量收缩期峰值流速(PSV)、舒张期末期流速(EDV),阻力指数(RI),以Philips IU22定量分析软件(QLAB)获得血管指数(VI),血流指数(FI)和血管血流指数(VFI)。结果:健康成人中指末节指掌血流信号丰富,可显示指掌侧固有动脉、甲床微动脉、指掌动脉弓及细小分支、各动脉之间细小交通支。系统性红斑狼疮患者血流信号明显减少,治疗后血流信号增多。健康成人中指末梢指掌动脉收缩期峰值流速和舒张末期流速均高于系统性红斑狼疮患者,治疗后患者均较未治疗者提高,差异均有显著性意义(P<0.01)。系统性红斑狼疮患者VI、FI及VFI均低于正常对照组(P<0.01),治疗后较未治疗者增高(P<0.05)。结论:高频超声结合彩色多普勒血流定量技术可以反映系统性红斑狼疮患者指端微血管改变,为临床了解系统性红斑狼疮微血管病变及评价治疗效果提供一种敏感、无创的新方法。Objective: To investigate the clinical value of high frequency ultrasonography in combination with quantitative studies of blood flow in the evaluation of fingertip vascular changes in patients with systemic lupus erythematosus (SLE). Methods:Of the 30 patients with SLE, 18 patients didn't have treatment and 12 patients were treated. 30 healthy subjects were included as control group. High frequency ultrasonography in combination with quantitative studies of blood flow were used to observe the configuration, distribution, number, peak systolic velocity (PSV), end diastolic velocity (EDV) ,vascular resistance index (RI) of digital palmar arteries in the last segment of right middle finger. The indexes in cluding vascularization index (VI), flow index (FI) and vascularization flow index (VFI) were calculated with Philips IU22 QLAB software. Results:In healthy subjects, rich blood supply could be revealed within the fingertips. Digital palmar propria arteries, micro-arteries of nail bed, digital palmar aterial arch and small inter-arterial collateral branches could be demonstrated. Signals of blood flow were markedly reduced in SLE patients, yet were increased after treatment. Both PSV and EDV of terminal digital palmar arteries in health control group were higher than that of SLE group,and the treated SLE patients al so showed higher PSV and EDV than that before treatment,significant statistical differences were existed (P〈0.01). VI, FI,and VFI in SLE patients were significantly lower than that of control group,and increased after treatment (P〈0.05). Conclusion:The micro-vascular changes could be assessed by high frequency ultrasonography in combination with quantita- tive studies of blood flow,which provided a sensitive and noninvasive approach for the evaluation of micro-vascular changes as well as treatment effects of SLE patients.
