摘  要:目的探讨多层螺旋CT三维重建在先天性髋关节脱位(CDH)的临床应用。方法对先天性髋关节脱位患者377例行双髋关节螺旋CT扫描,采用PHILIPS16排螺旋CT机经工作站进行表面遮盖显示(SSD),最大密度投影(MIP),多平面重建(MPR)。并测量股骨颈前倾角(FNA)、髋臼指数(AI)和头臼关系行三维直观显示和相关数据测量。结果377例患儿FNA均加大。最大90°,最小39.5°,平均43.73°±12.08°。AI均有不同程度加大,平均为35.3°±9.6°。髋臼壁不同程度发育不良与术中所见符合。结论多层螺旋CT三维重建对先天性髋关节脱位的手术方案制订具有重要的指导意义。Objective To evaluate the clinical application of three-dimension reconstruction(3D) using multi-slice spiral CT (MSCT) in diagnosing congenital dislocation of hip(CDH). Methods 377 patients with CDH were examined with spiral CT and three-dimensional reconstruction imaging was done before operation at 16-slice spiral CT (Philips). The 3D images included SSD, MIP and MRP which were processed on an independent computer workstation. The anteverted angle of femoral neck( FNA), acetabu- lum index(AI) and the relationship between the head of femur and acetabulum were measured or observed. Results The FNAs in all cases were all greater than those in normal, the greatest was 90° and the smaller was 39.5°, with average of 43.73°± 12.08°. The AIs were also increased, with average of 35.3°±9.6°. The acetabulums were hypogenesis in different degree. Conclusion MSCT 3D images are of significance in guiding the plan of treatment of CDH before operation.
