摘  要:目的:评价原发性高血压(EH)患者动态血压节律异常对利钠肽及左室质量的影响。方法:根据24h动态血压结果将65例EH患者分为杓形高血压组与非杓形高血压组,分别进行了血心房利钠肽(ANP)、脑钠肽(BNP)及超声心动图测定。结果:杓形高血压组与非杓形高血压组的ANP、BNP及左室质量指数(LVMI)均显著高于对照组(P<0.01);其中非杓形高血压组显著高于杓形高血压组(P<0.01);且ANP、BNP浓度与LV-MI呈正相关(R=0.45,0.67;P<0.01)。结论:EH患者血压昼夜节律异常者,心脏ANP分泌增加,与LVMI呈正相关性。Objective:To evaluate the effects of circadian rhythm on natriuretic and left ventricUlar mass with essential hypertension. Method: According to ambulatory blood pressure, 65 patients with EH were divided into two groups:dipper and non dipper. The atrial natriuretic peptide (ANP), brain natriutetic peptide (BNP) and left ventricular mass index (LVMI) were measured. Result: the ANP, BNP levels and LVMI were significant higher in patients of EH than health people (P〈0.01), and the diference between dipper and non dipper groups were statistically significant (P〈0.01). ANP and BNP positively correlated with LVMI in EH patients (R=0.45, 0. 673 P〈 0.01). Conclusion: Hypertensive patients with non dipper pattern show damage of target organs. ANP and BNP were elevated before cardiac dysfunction. The levels of natriuretic peptide could reflect the rhythm of blood pressure and the structural abnormality of left ventricle in EH patients.
