妊娠期高血压疾病孕妇血内皮素、一氧化氮、肿瘤坏死因子水平变化及意义

摘　　要：目的:探讨一氧化氮(NO)、内皮素-1(ET-1)和肿瘤坏死因子(TNF)含量在妊娠期高血压疾病中的变化,及其与发病之间的关系。方法:将我院产科分娩的96例妊娠期高血压疾病患者(妊娠期高血压疾病组),根据病情轻重分为妊娠期高血压组(39例),重度子痫前期组(53例)。采用酶联免疫吸附试验检测所有患者静脉和新生儿脐血血浆中ET-1,用放射免疫法检测血清中NO、TNF含量并与48例正常孕妇(正常妊娠组)作比较。结果:妊娠期高血压疾病组与正常妊娠组相比,母血与脐血中ET-1水平升高(P<0.01;P<0.05),NO水平降低(P<0.01;P<0.05),TNF水平升高(P<0.05),重度子痫前期组较妊娠期高血压组ET-1、TNF水平均升高(P<0.05),NO水平降低(P<0.05)。结论:妊娠期高血压疾病患者ET-1合成增加,NO释放减少,TNF分泌过多,三者调控失衡在妊娠期高血压疾病的发病中具有重要作用。Objective. To approach the variation of serum endothelin （ ET-1 ）, nitrogen oxide （NO） and tumor necrosis factor （TNF） in the course of hypertensive disorder complicating pregnancy. Methods： 96 of pregnant women with gestational hypertension in our hospital were divided into mild group （39 cases） and severe group （53 cases）, according to its degree. The content of ET-1, NO and TNF were detected by ELISA from blood sample of these 96 women and cord blood of their newbom. The data were compared with 48 normal pregnant women. Resuits： The content of ET- 1 and TNF in pregnant women with hypertensive disorder complicating pregnancy and their newbom cord blood were increased（ P〈 0.01 ; P〈 0.05） ,when compared to normal control, while the level of NO decreased （ P 〈0.01 ; P 〈0.05）. The level of ET-1 and TNF in women with severe preeclampsia were higher than those of gestational hypertension （ P 〈 0.05）, but NO level was lower in them. Conclusions： The increased synthesis of ET-1, lower release of NO, and over-secretion of TNF in the patients with hypertensive disorder complicating pregnancy play an important role in pathogenesis of pregnancy-induced hypertension disease.
