针刺结合刺络拔罐治疗脑卒中后肩手综合征疗效观察  

Observations on the Efficacy of Acupuncture Plus Pricking-cupping Bloodletting in Treating Postapoplectic Shoulder-hand Syndrome

摘　　要：目的观察针刺结合刺络拔罐和单纯针刺治疗脑卒中后肩手综合征(限I期患者)的临床疗效。方法将80例符合纳入标准的患者随机分为A、B两组,每组40例。A组采用针刺结合刺络拔罐治疗,B组单用针刺治疗,3个疗程后进行疗效评价。结果两组治疗后肩痛程度、肩痛频度及肩关节自主活动度改善均有明显效果(P<0.05,P<0.01),且A组均优于B组(P<0.01);治愈率A组为17.5%,B组为7.5%;显效率A组为27.5%,B组为17.5%;总有效率A组为92.5%,B组为70.0%。两组总有效率比较差异有统计学意义(P<0.05)。结论针刺结合刺络拔罐和单纯针刺治疗脑卒中后肩手综合征都有明显效果,但针刺结合刺络拔罐的疗效更优。Objective To compare the clinical efficacy of acupuncture plus pricking-cupping bloodletting with simple acupuncture in treating postapoplectic shoulder-hand syndrome （limited to the patients in the first period）. Methods Eighty patients meeting the inclusion criteria were randomly allocated to group A and group B, 40 cases each. Group A was treated with acupuncture plus pricking-cupping bloodletting and group B, with simple acupuncture. The therapeutic effects were evaluated after three courses of treatment. Results The degree of shoulder pain, the frequency of shoulder pain and the voluntary activity of the shoulder joint improved after treatment in both groups （P〈0.05, P〈0.01）, but the effect was better in group A than in group B （P〈 0.01）. The cure rateand marked efficacy rate were 17.5% and 27.5%, respectively, in group A and 7.5% and 17.5%, respectively, in group B. The total efficacy rate was 92.5% in group A and 70.0% in group B. There was a statistically significant difference in the marked efficacy rate between the two groups （P〈0.05）. Conclusion Both acupuncture plus pricking-cupping bloodletting and simple acupuncture have a marked therapeutic effect on postapoplectic shoulder-hand syndrome, but acupuncture plus pricking-cupping bloodletting has a better effect.
