能量多普勒定量检测甲状腺肿瘤血管生成的应用研究

摘　　要：目的:应用能量多普勒(CDE)在体外检测甲状腺肿瘤血管生成,并进行定量分析,期望发展一种无创性评价肿瘤血管生成方法,提高良恶性肿瘤诊断准确率。方法:入选经病理证实的35例甲状腺良性肿瘤(结节性甲状腺肿21例,腺瘤14例)和21例甲状腺癌病人术前均行能量多普勒(CDE)检查,包括CDE值、血流动力学参数、血流频谱。并对其差异进行分析。结果:良恶性肿瘤的CDE值及血流动力学参数(Vmax、RI)存在极显著差异(P<0.01);良恶性肿瘤的血流频谱特征存在差异:收缩期峰值位置存在极显著差异(P<0.01),舒张期始波位置存在显著差异(P<0.05)。结论:CDE能够在体外原位评价甲状腺肿瘤血管生成及其活性,对鉴别甲状腺良恶性肿瘤具有重要意义。Objective：To evaluate angiogenesis by color Doppler energy Imaging（CDE）in vitro and develop a noninvasive method for quantitative detecting angiogenesis of thyroid tumor and improve the accuracy of ultrasonic diagnosis in thyroid cancer.Methods：CDE was performed before surgery to search for blood flow signals in 35 thyroid benign masses and 21 cases of thyroid cancers,detectry the values of CDE,Vmax,RI and Doppler flow spectrum.The differences of data between the thyroid masses and thyroid cancers were analysed.Results：There were very significant difference in the values of CDE,Vmax and RI between the thyroid benign mass and cancer（P〈0.01）and there was significant difference between Doppler flow spectrum of the benign mass and cancers,both in the the peak of systolic period（very significant difference P〈0.01）and position of the beginning of diastolic period（P〈0.05）.Conclusion：CDE can show blood flow of the tumor fully,so it can evaluate angiogenesis in vitro and situ.It is important to distinguish the thyroid cancers from the benign ones.
