早产儿尿S100B蛋白测定对早期SEH-IVH的临床评价

摘  要:目的:探讨动态监测早产儿尿S100B蛋白对室管膜下及脑室内出血(SEH-IVH)的临床意义。方法:对住院早产儿84例于生后24h及120h留尿测定尿S100B蛋白,同时观察临床表现及头颅B超检查,对早产儿SEH-IVH进行早期评估。结果:SEH-IVH组(27例)尿S100B蛋白水平均高于对照组(44例),两组比较存在统计学差异(均P<0.05)。SEH-IVHⅠ级16例,SEH-IVHⅡ级8例,SEH-IVHⅢ级3例,尿S100B蛋白随SEH-IVH分级增加,差异存在统计学意义(均P<0.05)。SEH-IVH27例中13例在生后24h时尿S100B蛋白即升高,10例为生后120h检测时升高,4例变化趋势平稳;对照组44例S100B蛋白动态检测呈下降趋势。84例早产儿其中痊愈54例,好转27例,恶化及死亡3例,病情恶化及死亡者尿S100B蛋白水平最高,24h可高达(8.32±1.90)μg/L。结论:动态监测尿S100B蛋白水平结合临床表现及影像学检查对早产儿SEH-IVH可早期做出更客观的评价,有助于判断预后。Objective: To investigate the clinical value of measurement of urinary S100B protein concentrations in preterm infants with SEH-IVH. Methods: The urinary S100B protein concentrations were measured in 84 preterm infants using ELISA at 2 predetermined time points (24 hours and 120 hours after birth ).At the same time ,routine clinical observations, neurologic patterns, ultrasound screens were recorded. Results: S100B protein concentrations were significantly higher in samples collected at 2 monitoring time points from preterm infants with SEH- IVH (n=27) than in samples from those without SEH- IVH (P〈0.05) ;The S100B protein concentrations were different among SEH-IVH Ⅰ, SEH-IVH Ⅱ and SEH-IVH Ⅲ (P〈0.05), and they were also different among preterm infants with different prognosis. In infants those died or deteriorated, the S100B levels in urine were significantly higher than those of others (P〈0.05).Conclusion: Longitudinal S100B protein measurements in urine combined with clinical presentation and ultrasound screen are helpful to identify preterm infants with SEH-IVH and are beneficial to predict prognosis.
