摘  要:目的探讨组织多普勒(DTI)Tei指数评价慢性肺心病患者右心室功能的临床价值。方法超声观察56例慢性肺心病患者(病例组)及40例正常人(对照组)常规超声指标,超声指标包括:右心室横径、右心室前壁厚度、肺动脉主干内径、根据三尖瓣反流估测肺动脉收缩压。超声心动图DTI条件下,于心尖四腔观获得右心室游离壁近三尖瓣瓣环位点组织多普勒频谱,测量舒张晚期Am波终止至下一舒张期Em波起始的间期(a)和收缩期Sm波的间期(b),通过(a-b)/b计算Tei指数。结果病例组较对照组指数明显延长(P<0.01)。结论右心室Tei指数是反映右心整体功能的较敏感指标,DITTei指数可综合评价肺心病患者右心室整体收缩舒张功能。Objective To investigate the clinical value of Tei index of Doppler tissue imaging (DTI) in the assessment of right ventricular function in patients with chronic pulmonary heart disease. Methods 50 patients with chronic pulmonary heart disease and 40 healthy subjects were involved in the study. Some routine ultrasonic parmneters were measured and compared, including transverse diameter of right ventricle, thickness of anterior wall of right ventricle, inner diameter of main pulmonary artery and systolic pressure estimated according to tricuspid regurgitation. TDI velocity prof'des of tricuspid annulus at right ventricular free were acquired at the apical four chamber view. The time interval from the end of diastolic Am wave to the early diastolic Em wave (a) and the duration of systolic phase Sm wave (b) were measured and Tel index was calculated from the equation Tei index = (a-b)/b. Results Tei indexes of patients with chronic pulmonary heart disease were increased significantly, as compared with those of the healthy subjects( P 〈 0.01) .Conclusion Right ventricular Tei index is a quite sensitive indicator for right ventricular global function. Tei index of Dopple tissue imaging can evaluate right ventricular global systolic and diastolic function in patients with chronic pulmonary heart disease.
