摘  要:目的:探讨美托洛尔应用于64层螺旋CT冠状动脉成像心率控制的量效关系,以期优化工作流程。方法:选择2006年10月~2008年5月心率在85~100次/min之间的受检者1 000例,按预约检查先后顺序随机交替分为观察组和对照组各500例,在造影检查前与受检者充分沟通后,对照组受检者遵医嘱首次口服美托洛尔50 mg,半小时无效后追加25 mg,直至心率降至70次/min;观察组受检者遵医嘱一次口服美托洛尔100 mg。比较两组受检者心率控制时间、检查中心率波动幅度及药物不良反应发生率之间的差异。结果:观察组受检者心率控制时间明显比对照组短,检查中心率波动>10次/min发生率明显比对照组低(P<0.01),差异均有极显著统计学意义;两组受检者药物不良反应发生率比较差异无统计学意义(P<0.05)。结论:美托洛尔应用于64层螺旋CT冠状动脉成像控制心率,对心率在85~100次/min的受检者,一次口服美托洛尔100 mg,安全有效,可明显缩短受检者心率控制时间,优化工作流程,使设备、人力资源得到最大程度的发挥。Objective : To explore the dosage - effect of metoprolol applied in 64 - slice spiral CT coronary angiography to control heart rate in the hope of optimizing the work flow. Methods: 1 000 patients with heart rate between 85 - 100 bpm were randomly divided into experimental group and control group. In the control group, they took 50 mg of metoprolol at first, and added another 25 mg dose if the heart rate couldn't fall to 70 bpm. While in the experimental group, they took 100 mg only once. Compared the time of heart rate control, heart rate fluctuation and side effect of drug between two groups. Results : There was remarkable sigrtifican senses between two groups,the time of heart rate control in experimental group was shorter than control group and the fluctuation 〉 10bpm was lower than control ( P 〈 0.01 ), there was no statistic significance difference of side effect between two groups. Conclusion : Metoprolol Was safely applied as a heart control in 64 - slice spiral CT. It could reduce the time of heart rate control to optimize the work flow.
