游离胸脐皮瓣修复前足脱套伤

摘　　要：目的:探讨游离胸脐皮瓣移植修复前足脱套伤的临床效果。方法:前足脱套伤患者9例,男7例,女2例;年龄20~49岁,平均年龄37岁。对9例患者均采用胸脐皮瓣游离移植修复创面。创面面积15cm×6 cm^30 cm×12 cm。结果:移植皮瓣全部成活,4例皮瓣臃肿,需再次手术整形。其余病例外观和足部功能恢复满意。结论:胸脐皮瓣为腹壁下动脉供血,血运丰富,血管解剖恒定,血管蒂长,可切取面积大且易于切取,适宜修复前足脱套伤,尤其适用于有部分足底皮肤缺损者。Objective： To discuss the clinical effect about flyback avulsion injury of skin in foot that repaired by free thoraco- umbilicus flaps. Methods： Nine patients including seven males and two females, from twenty to forty - nine years old, with an average of thirty - seven years. Nine cases of flyback avulsion injury of skin in foot were treated with free thoraco-umbilicus flaps. The defects was 15 cm× 6 cm-30 cm× 12 cm. Results： All thoraco- umbilicus flaps survived. Flaps was fat and clumsy in four cases which needs to be operated again. The appearance and functions in other cases were satisfactory. Conclusion： Thoraco - umbilicus flap is nourished by the inferior epigastric artery, which was highly vascularized, with a constant vascular anatomy and a long vascular pedicle, so the dissection of the flap was easy, and larger area of flaps could be obtained. The flap is suitable for the repair of flyback avulsion injury of skin in foot, especially, it is suitable for the repair of avulsion injury of skin in sole of foot.
