摘  要:目的:探讨2型糖尿病家系正常糖耐量一级亲属血清高敏C反应蛋白(hs-CRP)水平变化及其临床意义。方法:收集2型糖尿病家系34个,51例正常糖耐量一级亲属为研究组(FDR组),46例无糖尿病家族史者为正常对照组(NC组)。检测两组血清hs-CRP、空腹血糖(FPG)、空腹胰岛素(FINS)、血脂和血尿酸(SUA)水平。采用稳态模型法计算胰岛素抵抗指数(HOMA-IR)。结果:与NC组比较,FDR组的血清hs-CRP(P﹤0.05)、FPG(P﹤0.01)、FINS(P﹤0.01)显著增高。相关分析表明,FDR组的hs-CRP水平与FPG、FINS和HOMA-IR相关。多元逐步线性回归分析显示,收缩压、SUA、甘油三酯是影响hs-CRP的独立危险因素。结论:2型糖尿病家系正常糖耐量一级亲属体内存在慢性炎症反应,增高的hs-CRP水平可能早期参与了2型糖尿病的发生。Objective To explore the high sensitive C-reactive protein (hs-CRP) level and its clinical significance in nonnoglycemic first-degree relatives of type 2 diabetic family. Methods 34 families of type 2 diabetic were recruited. The study group comprised 51 normoglycemic first-degree relatives of type 2 diabetes, the normal control group comprised 46 spouses of probands or siblings without diabetes mellitus or diabetic family history. The serum levels of hs-CRP, fasting plasma glucose (FPG), fasting serum insulin (FINS), lipid profile and blood uric acid (UA) were measured. Insulin resistance was estimated using Homa model. Results Compared with normal control group , serum levels of hs-CRP (P 〈 0.05 ), FPG(P 〈 0.01 ), FINS(P 〈 0.01 ) in normoglycemic first-degree relatives were significantly increased. The levels of hs-CRP were correlated to FPG, FINS and HOMA-IR in the first-degree relatives by correlative analysis. Stepwise lineal regression analysis showed that systolic blood pressure (SBP), UA and triglyceride (TG) were the independent risk factors of hs-CRP. The levels of hs-CRP were significantly higher and closely associated to FPG, FINS and HOMA-IR. Insulin resistance existed in normoglycemic first-degree relatives of type 2 diabetic family. SBP, SUA and TG were the independent risk factors of hs-CRP. Conclusions There existed chronic inflammatory reaction in the normoglycemic first-degree relatives. Increased levels of hs-CRP may early participate in occurrence of type 2 diabetes.
