摘 要:【目的】了解小儿人类偏肺病毒(human metapneumo-virus,hMPV)感染CD4+、CD8+细胞,IL-4、IFN-γ和lgE变化情况,探讨其与喘息性疾病之间关系。【方法】225例下呼吸道感染患儿分别检测hMPV特异性基因,外周血CD4+、CD8+细胞,IL-4、IFN-γ和lgE。【结果】①共检出hMPV29例(12.9%),伴喘息症状者hMPV检出率(15.2%)明显高于不伴喘息症状者(7.5%)(P<0.01);②毛细支气管炎、喘息性支气管炎、哮喘发作组CD4+、CD8+,IFN-γ下降,CD4+/CD8+,IL-4I、gE、IL-4/IFN-γ升高(P<0.05或<0.01);③hMPV(+)和hMPV(-)组同对照组比较,CD4+、CD8+、IFN-γ下降,CD4+/CD8+I、L-4、IgE升高(P<0.05),但两组间比较差异无显著性(P>0.05);④hMPV感染患儿IL-4、IL-4/IFN-γ与IgE呈正相关,IFN-γ与IgE呈负相关(P<0.01)。【结论】hMPV感染患儿T细胞亚群,细胞因子改变明显,免疫功能紊乱,且免疫功能紊乱可能是导致喘息的重要原因之一。[Objective] To investigate the changes of CD4^+ ,CD8^+ ,IL-4,IFN-γ and IgE in young children infected by Human Metapneumo-virus(hMPV), and the relation between hMPV and wheezing. [Methods] The special gene of hMPV and the level of CD4^+ .CD8^+ .IL-4.IFN-γ.lgE were checked in 225 LRTIs. [Results] ①Twenty nine(12.9%) were hMPV positive in all, the hMPV rate( 15.2%) of those with wheeze was higher than those(7.5%)with no wheeze(P〈0.01). ②The levels of CD4^+ , CD8^+ and IFN-γ declined, while The levels of CD4^+/CD8^+ , IL-4, IgE and IL-4/IFN-γ rised(P〈0.05 or G0.01)in children with bronchiolitis, wheezing bronchitis or asthma break out.③The levels of CD4^+ , CD8^+ and IFN-γ declined, while the levels of CD4^+/CD8^+ ,IL-4,IgE and IL-4/IFN-γ rised(P〈0.05 or 〈0.01 )in both hMPV( + ) and hMPV( - ) groups,there was no significant between hMPV( + ) and hMPV(-) groups (P〈0.05). ④ The level of IgE was positive relatived with those of IL-4,IL-4/IFN-γ(P〈0.01),while it was negative relatived with IFN-γ(P〈0.01). [Conclusions] The levels of T-lymphocyte Subsets (CD4^+ ,CD8^+ ) and Cytokines( IL-4,IFN-γ, IgE) are changed greatly in young children infected by hMPV, and its immunity function is disorded; hMPV infection is an important risk factor in wheezing.
