摘  要:【目的】讨论网织红细胞平均血红蛋白含量(CHr)对小儿铅中毒的诊断价值。【方法】用拜耳AD-VIA120全自动血液分析仪检测75名健康儿童和75例临床诊断为铅中毒患儿的外周血细胞和网织红细胞血红蛋白含量,用原子吸收光谱仪进行血铅测定,将所得数据进行统计学分析。【结果】铅中毒儿童单个网织红细胞平均血红蛋白(CHr)、白细胞(WBC)、血小板(PLT)明显低于健康儿童,而网织红细胞计数(Ret)明显高于健康儿童。网织红细胞平均血红蛋白含量(CHr)与血铅呈高度负相关(r=-0.806,P<0.001)。网织红细胞计数(Ret)与血铅呈高度正相关(r=0.755,P<0.001)。ROC曲线显示,铅中毒时的CHr面积为0.89,Ret为0.87,差异有显著性(P<0.01)。【结论】CHr作为诊断铅中毒的指标,有重要的临床价值。[Objective] To discuss the clinical value of reticulocyte hemoglobin content in children's lead poisoning. [Methods] Blood cell and CHr were measured by automated hematology analyzer (ADVIA120) in 75 healthy children and 75 lead poisoning children. Blood lead levels were tested by atomic absorption spectrographic method. [Results] CHr, WEC, PLT were lower in lead poisoning children than those in healthy children, Ret was higher in lead poisoning than healthy children. It was a good negative correlation between CHr and blood lead level(r=-0. 806 ,P〈0. 001). Ret and blood lead level show a good positive correlation(r=0. 755 ,P〈0. 001 ). Receiver operator characteristic curve (ROC) analysis in diagnosis of lead poisoning that the area under the curve for CHr and Ret were 0.89 and 0. 87, respectively (P〈 0.01). [Conclusion] Conclusion CHr has important clinical value in diagnosing children's lead poisoning.
