摘 要:目的探讨血小板膜糖蛋白GPⅡb/Ⅲa和血浆纤维蛋白原(FIB)对诊断和监测子痫前期和子痫的临床意义。方法2007年1月至2008年2月于吉林大学第二医院采用流式细胞术和ACL血凝仪检测子痫前期及子痫、正常孕晚期和正常未孕妇女全血中GPⅡb/Ⅲa和血浆FIB水平。结果(1)血小板膜糖蛋白GPⅡb/Ⅲa水平:分娩前子痫前期及子痫组(1.98±0.53)%比正常未孕组(0.46±0.12)%、正常孕晚期组(0.54±0.19)%均明显增高,差异有统计学意义(P<0.01);分娩前重度子痫前期及子痫组(2.60±0.63)%与正常未孕组、正常孕晚期组、轻度子痫前期组比较差异有统计学意义(P<0.01);分娩后各组之间差异均无统计学意义(P>0.05)。(2)血浆FIB水平:正常孕晚期组(4.87±0.98)g/L、轻度子痫前期组(5.89±1.25)g/L及重度子痫前期及子痫组(4.67±1.36)g/L与正常未孕组(3.09±1.10)g/L比较均明显增高,差异有统计学意义(P<0.01);重度子痫前期及子痫组比轻度子痫前期组明显降低(P<0.05),与正常孕晚期组比较差异无统计学意义(P>0.05)。结论重度子痫前期及子痫与轻度子痫前期患者比较,全血中GPⅡb/Ⅲa表达水平增高及血浆FIB水平降低,联合检测全血中GPⅡb/Ⅲa和FIB,可作为诊断监测指标。Objective To detect the clinical significance of levels of blood GPⅡb/Ⅲa and serum FIB in diagnosing and monitoring pre-eclampsia and eclampsia. Methods Fibrinogen(FIB) and GPⅡb/Ⅲa a were measured in the normal non-pregnany women, normal late-pregnancy women and pre-eclampsia and eclampsia women. The levels of FIB in the serum were detected by hemagglutinin' s appearance, and the GPⅡb/Ⅲa of whole blood was detected by flow cytometry, Results The levels of GPⅡb/Ⅲa in the blood : the levels of GPⅡb/Ⅲa in pre-eclampsia/eclampsia women in pregnancy ( 1.98 ± 0. 53 ) % were higher than non-pregnancy women ( 0. 46 ±0. 12 ) % and normal late-pregnancy women ( 0. 54 ± 0. 19) % ( P 〈0. 01 ) ; Severe pre-eclampsia/eclampsia in pregnancy( 2.60 ±0. 63 ) % was higher than non-pregnancy, normal late-pregnancy and mild pre-eclampsia( P 〈 0. 01 ). There was no significance between three groups in-post-delivery. The levels of FIB in serum:the levels of FIB of the normal late-pregnancy groups (4. 87 ±0. 98 ) g/L, the mild pre-eclampsia(5.89 ± 1.25) g/L and the severe pre-eclampsia/eclampsia groups (4. 67 ± 1.36) g/L were much higher than the non-pregnancy groups( 3.09 ± 1.10 ) g/L (P 〈 0. 01 ). The levels of FIB of severe pre-eclampsia/eclampsia groups were lower than mild pre-eclampsia ( P 〈 0.05 ). There was no difference between severe pre-eclampsia/eclampsia and normal late-pregnancy. Conclusion The levels of GPⅡb/Ⅲa in the blood are raised and the levels of FIB in serum are lower in the severe pre-eclampsia/eclampsia and mild pre-eclampsia. It is one of important indexes to detect the GPⅡb/Ⅲa and the levels of FIB in the blood.
