检测脓毒症大鼠血碱性磷酸酶水平对预后判断的意义

摘　　要：目的:观察脓毒症大鼠模型血碱性磷酸酶(ALP)水平变化,揭示脓毒症时血ALP水平对预后的意义。方法:采用盲肠结扎穿孔术制备脓毒症大鼠模型,通过控制穿孔数目和是否放置引流条来控制病情严重程度,测定血ALP、白蛋白(ALB)、钙(Ca2+)、总胆红素(TBIL)和C反应蛋白(CRP)水平。结果:脓毒症轻症组和重症组大鼠血ALP、ALB和Ca2+水平均低于对照组(P<0.01),血TBIL和CRP水平均高于对照组(P<0.01)。重症组大鼠血ALP、ALB和Ca2+水平低于轻症组(P<0.01),血TBIL水平高于轻症组(P<0.01)。血CRP水平差异无统计学意义(P>0.05)。血ALP水平与ALB和TBIL相关(P<0.01),与Ca2+和CRP无相关性(P>0.05)。血ALP水平与病情轻重程度负相关(r=-0.933,P<0.01)。结论:血ALP水平与脓毒症感染严重程度和器官损伤程度负相关,病情越重,血ALP水平越低,可用于判断脓毒症预后。Objective： To observe the serum alkaline phosphatase （ALP） level in rat model with sepsis, and its significance on the prognosis in sepsis thereof. Methods ：The rat model with sepsis was established with cecal ligation and puncture. The levels of serum ALP, albumin （ALB）, Ca^2＋, total bilirubin （TBIL） and C-neactveprotein （CRP） were measured. Results： The levels of serum ALP, ALB and Ca^2＋ were lower in mild and severe group than those of control group （P 〈 0.01）.The levels of serum TBIL and CRP were higher than that of control group （P 〈 0.01）. The levels of serum ALP, ALB and Ca^2＋ were lower in the severe sepsis group than those of mild group （P 〈 0.01）. The serum TBIL levels were higher in the severe sepsis group than that in the mild group （P 〈 0.01）. There was no statistical difference in serum CRP level between the severe group and the mild group （P 〉 0.05）. The level of ALP was correlated with ALB and TBIL （P 〈 0.01）. There was no statistical correlation between the levels of ALP and Ca^2＋, and ALP and CRP （P 〉 0.05）. The serum ALP level was negatively correlated to the severity of sepsis（r = -0.933, P 〈 0.01）. Conclusion： The serum ALP level was negatively correlated to the severity of sepsis. The lower the ALP level was, the more severe the sepsis was. The serum ALP level was a reliable marker for the prognosis of sepsis.
