PPH术与外剥内扎术治疗环形混合痔临床对比观察

摘　　要：目的比较吻合器痔上黏膜环形切除术(PPH)与外剥内扎术治疗环形混合痔临床效果。方法将156例环形混合痔患者随机分为A组(PPH组)和B组(对照组)各78例,分别进行吻合器痔上黏膜环切术和外剥内扎术。比较两组手术时间、创面愈合时间、住院时间及其术后疼痛指数、肛门功能、术后并发症发生情况。结果两组患者术后原有症状均得到了有效控制。手术时间、创面愈合时间、住院时间及其术后疼痛指数、肛门功能A组均优于B组。肛管压力测定A组手术前后无变化,B组术后明显降低。结论与外剥内扎术相比,PPH术安全、有效、手术时间短、住院时间短、恢复快、并发症少。Objective To compare the clinical effects of PPH and Milligan-Morgan for the treatment of ring mixed hemorrhoid ring mixed. Methods 156 ring mixed hemorrhoid cases were randomly divided into group A（PPH group） and group B（control group） of the 78 cases, respectively of PPH and Milligan-Morgans. The surgery time, wound healing time, hospital stay time and postoperative pain index, anal function, the occurrence of postoperative complications were compared in two groups. Results All patients original symptoms had been effectively controlled. Surgery time, wound healing time, hospital stay time and postoperative pain index, anal function in group A were better than in group B. In group A anal pressure before and after operation did not change, but significantly decreased in group B. Conclusion Compared with Milligan-Morgan, PPH is more and safe effective, and hemorrhodectomy has shorter surgery time, loss hos- pital time, rapider recovery, fewer complications.
